On May'14, 1990, Houston Lighting & Powet Company (HIAS) notified the NRC of a reportable' event due to an inadequate surveillance performed on a Control Room toxic gas analyzer as specified in Technical Specification 4.3.3.7.. Operability of the analyzer was not affected. This event had no adverse impact on the health and safety of the public. HIAP submits the attached Licensee Event ~ Report (LER 90 009) in accordance with 10CFR50.73.

At 0900 hours0.0104 days <br />0.25 hours <br />0.00149 weeks <br />3.4245e-4 months <br />, operations personnel determined that Technical Specification required surveillances for toxic gas monitor XE-9325 had been performed incorrectly from 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br /> on May 11, 1990, until 0400 hours0.00463 days <br />0.111 hours <br />6.613757e-4 weeks <br />1.522e-4 months <br /> on May 14, 1990. The printer associated with analyzer XE-9325 failed to update which resulted in non licensed operators recording erroneous data for approximately 56 hours6.481481e-4 days <br />0.0156 hours <br />9.259259e-5 weeks <br />2.1308e-5 months <br />. The causes of the event were less than adequate training on the performance of the channel check and less than adequate detail in the operator logging procedure. Corrective actions include the training of licensed and non licensed operators and the revision of the operator logging procedures.

On May 14, 1990, Unit 1 was in Mode 5. At 0900 hours0.0104 days <br />0.25 hours <br />0.00149 weeks <br />3.4245e-4 months <br />, operations pcrsonnel determined that the 12 hour1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> channel check surveillance for toxic gas analyzer (XE 9325) required by Technical Specification 4.3.3.7 was exceeded because the surveillance was performed incorrectly. The NRC was notified of this Technical Specification violation at 1258 hours0.0146 days <br />0.349 hours <br />0.00208 weeks <br />4.78669e-4 months <br /> on May 14, 1990.

At 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> on May 11, 1990 a modification was completed on the power supply of toxic gas analyzer XE 9325. A post maintenance test was performed successfully which verified the accuracy of channel sensors, trip setpoints, alarms, signal processing equipment and remote displays. The toxic gas analyzer was placed in service and declared operable at 1652 hours0.0191 days <br />0.459 hours <br />0.00273 weeks <br />6.28586e-4 months <br />.

The associated printer for each toxic gas analyzer is programmed to print data every eight hours in order to complete the 12 hour1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> channel check surveillance required by Technical Specification 4.3.3.7. The channel check is satisfied by recording the appropriate data from the printouts once per eight hour shift. The programmed printout was generated with the appropriate data as expected, at 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br /> on May 11, 1990.

The printer for toxic gas analyzer XE-9325 failed to generate the preset programmed printout between 0400 hours0.00463 days <br />0.111 hours <br />6.613757e-4 weeks <br />1.522e-4 months <br /> on May 12, 1990, and 0400 hours0.00463 days <br />0.111 hours <br />6.613757e-4 weeks <br />1.522e-4 months <br /> on May 14, 1990. The cause of the printer failure could not be determined. Each non licensed operator that performed the channel checks following the printer failure recorded data from the printout of May 11, 1990 to satisfy the surveillance but did not recognize that the data was invalid.

On May 13, 1990, at approximately 1430 hours0.0166 days <br />0.397 hours <br />0.00236 weeks <br />5.44115e-4 months <br />, after performance of the channel check it was discovered that the printer for XE-9325 had not updated since May 11, 1990. The operator was distracted by other priority work and failed to correct the operator logs or report the abnormality to the supervision.

On May 14, 1990, it was discovered that the printer was not updating.

Maintenance was contacted and the printer wheel was freed. The printout for May 14, 1990 at 0400 hours0.00463 days <br />0.111 hours <br />6.613757e-4 weeks <br />1.522e-4 months <br /> was obtained and the channel check was successfully performed. Review of the printouts from May 11, 1990 and May 14, 1990, revealed that toxic gas analyzer XE 9325 was functional for the duration of this event.

The causes of this event are:

1. Less than adequate training of non-licensed operators in the performance of the channel check surveillance on the toxic gas analyzers. Non licensed operators had not been instructed to verify date and time on the printouts for each analyzer surveillance.

2. Less'than adequate detail in the operator logging procedure. The logging procedures only required that data be taken from the printout and logged.

ANALYSIS OF EVENT:

This event.is repectable pursuant to 10CFR50.73(a)(2)(1). The surveillance required by Technical Specification 4.3.3.7 was incorrectly performed for approximate 1-, fifty six (56) hours. No toxic gas releases were reported for the duration of this incident.

This event had no effect on the health and safety of the public. The printout obtained on May 14 provided evidence that the analyzer continued to take samples. In the event of a toxic gas release the toxic gas analyzer would have initiated an ESF actuation and placed the Control Room Ventilation System in'the recirculation mode.

CORRECTIVE' ACTION:

The following corrective actions will be taken to prevent recurrence of the event:

1. Non-licensed operators will receive training on the performance of the channel check surveillance on toxic gas analyzers. The training will also stress the importance of Technical Specification compliance. This action will be comnleted by November 30, 1990.

2. A discussion of this event will be incorporated into the next cycle of licensed and non-licensed operator requalir'ication training.

This action will be completed by November 30, 1990.

3. The operator logs will be revised. This action will be completed by June 30, 1990.

4. Night orders will be issued directing operations personnel to verify the time and date on the printouts prior to performing the channel check surveillance on the toxic gas analyzers. This will be completed by June 15, 1990.
