On December 5, 1989, Unit I was in Mode 1 at 100 percent power. During a

, review of completed work packages which replaced the inside containment check i valves on the Train C Component Cooling Water System, an engineer discovered that an ASME Section XI pressure test had not been performed prior to returning the system to service. This condition was reported to the control room on 0930 on December 6, 1989. The system was declared inoperable.and the test was successfully completed at 1445 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.498225e-4 months <br />. This condition resulted in a violation of Technical Specification 3.7.3 since the Train C Component Cooling System was technically inoperable from September 9, 1989 to December 6, 1989 when the testing was completed on the valves. The causes of this event were that the test control process did not clearly delineate that all required tests were completed prior to return of a system to service, the procedure for test control did not adequately define the responsibility of the Test Coordinator, and operations personnel were not adequately trained in the procedure. The procedures and forms which govern the test control process will be revised to include specific signoff of each completed test prior to return of a system to service and to define the responsibility of the Test Coordinator. Instruction in post maintenance test control will be incorporated in licensed operator training. This event has been discussed in a bulletin to plant personnel. ,

During a review of completed work packages which replaced two inside containmentfcheck valves on the Train C Component Cooling Water System,Lan engineer' discovered that'an ASME Section XI pressure test required by Technical Specification 4.0.5 had not been performed prior to returning the system to service. The Shif t Supervisor was informed at 0930 hours0.0108 days <br />0.258 hours <br />0.00154 weeks <br />3.53865e-4 months <br /> on December 6,1989. and the . ,

affected valves were declared inoperable. The pressure test was subsequently.

performed at 1445 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.498225e-4 months <br />.and the valves were declared operable. The NRC was notified of this Technical Specification violation at 0615 hours0.00712 days <br />0.171 hours <br />0.00102 weeks <br />2.340075e-4 months <br /> on December 7, 1989.

Train C Component Cooling Water System inside containment check valves'CC-0198 and CC-0183 were replaced on August 31, 1989 and September 1, 1989, respectively. The Test Control Form for each valve specified.the appropriate i ASME Section XI tests. The Local Leak Rate Test was performed on each valve and the Test Control ~ Form was forwarded to Plant Operations for performance of  ;

an Operational Leak Check. A plant operator then completed the Operational Leak Check. Since Plant Operations was designated as the test coordinator for this activity, the licensed Unit' Supervisor then reviewed the test package and indicated that all testing was satisfactorily completed. However, he did not notice that the ASME Section XI Code Pressure Test was not complete. The system was declared operable on September 10, 1989. This condition was not detected until review of the test package by engineering on' December 5, 1989, s CAUSE OF OCCURRENCE:

l l The following causes of this event have been. identified:

! 1. The post maintenance test control form does not summarize test results in such a way that operations personnel can readily determine that all required tests are completed.

2. The procedure for test performance control did not adequately define responsibility'for completion and review of tests.

ANALYSIS OF EVENT:-

Technical Specification 4.0.5 requires that ASME Code Class 1, 2 and 3 valves.

be subjected to inservice inspection and. testing as defined in Section XI of

.the ASME Boiler and Pressure Vessel Code. Failure to perform the required ASME Section XI Code Pressore Test.resulted in one Component Cooling Water System check valve being technically inoperable from September 9, 1989.to December 6, 1989 and the other Component Cooling Water System check valve being technically inoperable from September 10, 1989 to December 6, 1989.-

This is a violation of Technical Specification 3.7.3 which is reportable pursuant to 10CFR50.73(a)(2)(1)(B).

The Component Cooling Water System was not operated in a degraded condition since a satisfactory Local Leak Rate Test was performed on each valve, the ,

valve operability test confirmed' proper system flow rates, and a visual-leak l check was performed prior to returning the system to service. Satisfactory. j performance of the Section XI Code Pressure Test on December 6,'1989 confirmed  ;

that the check valves had been operable. This condition did'not result in any j additional risk to the health and safety of the public. ,

i CORRECTIVE ACTION: i i

The following corrective actions are being taken as a result of this events j l

1. The Test Control Form and accompanying procedure will be revised to -j include requirements for individual signoff of each post maintenance i test prior to returning a system to service and to better define the responsibility of the Test Coordinator. This action will be completed }

by April 15, 1990.

l 2. A bulletin has been issued to operations and maintenance personnel ]

regarding the need to ensure that all required testing is completed- 1 prior to returning a system to service.

3. Supplemental procedure briefings will be provided to operations  !

personnel in the requirements for post maintenance test control by l' February 28, 1990.

4. The requirements of the revised procedure for post-maintenance testing control will be taught to hot license candidates and in requalification training for Licensed Operators by September 1, 1990.
