Attachment to Monthly Operating Report for October 1989 Limerick Generating Station l Unit 1

, October 1 through October 31, 1989 I. Narrative Summary of Ooeratina Exoeriences  ;

Limerick Unit 1 began the month of October in Operational l i Condition (OPCON) 1 (operations), at a nominal 100% of rated thermal power. On October 12, reactor power was reduced to approximately 90% to support a control rod pattern adjustment.

Reactor power was restored to 100% on October 13. On October i 19, reactor power was reduced to 81% following discovery of l an Electro Hydraulic Control (EHC) leak. The leak was l

contained and a control rod pattern adjustment made. Reactor power was increased to 100% on October 20. Unit i ended the month of October in OPCON 1 at a nominal 100% of rated thermal power. '

Operational events that occurred during the month of October included:

On October 19, reactor power was reduced by 200 MWe -

after an EHC leak was discovered on a pipe weld for the No. 2 main turbine control valve. Maintenance personnel installed a temporary clamp on the pipe i

to prevent the leak from getting larger. Drain tubing was also insta31ed to divert the EHC fluid into a drum in crdar to prevent admission of EHC fluid into the ?,dwaste System. A camera was set up so that the leak could be monitored. Upon

, completion of this work, a control rod pattern I adjustment was made and reactor power was increased back to 100%.

On October 22, the Main Control Room (MCR) was placed in the Chlorine Isolation Mode following receipt of a Toxic Gas alarm due to the 'A' Toxic Gas Analyzer detecting a concentration of vinyl chloride greater than 10 ppm in the MCR air intake plenum. Chemistry personnel sampled the intake plenum and no significant concentrations of toxic chemicals were detected. A local chemical company was contacted and reported that incinerator problems had occurred. After ensuring that the problem was resolved, the MCR chlorine isolation was reset and MCR HVAC restored to normal.

II. Challences to Main Steam Safety Relief Valves There were no challsnges to the Main Steam Relief Valves during the month of October.

3892386830 Docket No. 50-353 Attachment to Monthly Operating Report for October 1989 Limerick Generating Station Unit 2 October 1 through October 31, 1989 I. Narrative Summary of Operatina Exoeriences Limerick Unit 2 began the month of October in Operational Condition (OPCON) 4 (cold shutdown) with a Maintenance outage in progress. The outage was completed and start-up activities commenced on October 8. Unit 2 entered OPCON 1 on October 9 and reactor power was increased to 50% by October 12. On October 20, reactor power was raised to 65%, and again to 70%

on October 22 to support start-up testing. On October 23, reactor power was decreased to 55% when the '2A' recirculation pump was tripped and restarted as part of further start-up testing. Reactor power was then increased to 69% on October 24 and both recirculation pumps (2A and 2B) were tripped for recirculation pump coast down time testing. The '2A' recirculation pump was restarted and reactor power was increased 40%. The '2B' recirculation pump was restarted on-October 25 following maintenance and power was raised to 67%

by October 26. On October 27, a 75% control rod line was established and power was increased to 73%. Unit 2 ended the month of October in OPCON 1 (operations) at a nominal 73% of rated thermal power.

Operational events that occurred during the month of October included:

On October 13, the HPCI exhaust line drain pot was found to be draining improperly. This was due to a clog in the t- flow orifico drain line to the barometric condenser. The drain pot was filling up due to steam leaking past the steam inlet valve seat. HPCI was declared inoperable and the flow orifice was unclogged. HPCI was then placed in service to verify operability.
