SUPPLEMENTAL INFORMATION FOR LER NP-33-77-5 V
DATE OF EVENT: May 13, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: The channel check requirements of Table 4.3-2 of Technical Specification 4.3.2.1.1 were not satisfied.
Conditions _ Prior to Occurrence: The plant was in Mode 6.
Descriotion of occurrence: ST 5099.01 (Shif t Channel Check) was being run when it was discovered the channel check requirements of Table 4.3-2 of Technical Specification 4.3.2.1.1 were not all being checked, specifically la " Containment Radiation High" of Instrument Strings and 2a " Incident Level One Containment Iso-lation" of Output Logic.
Designation of Apparent Cause of Occurrc ;cji: A procedural error (deficiency) was determined to have caused the non-compliance with Technical Specification 4.3.2.1.1.
The procedure did not include all of the necessary channel check (N.,
reading sheets.
Analysis of Occurrence:
Since ST 5031.01 (SFAS Monthly Test) verified that the SFAS was operable, no threat to the health and safety of the public or station personnel occurred.
Corrective Action,:
Shif t Reading Sheets for Mode 6 of ST 5099.01 were revised on May 8, 1977 and May 11, 1977 to include and satisfy channel check require-ments of Table 4.3-2, Technical Specification 4.3.2.1.1.
Failure Data:
No previous similar events have occurred.
