On November 16, 1978, Unit Two Reactor Building to Suppression Chamber Vacuum Breaker Valve (2-1601-20A) failed to open while performing quarterly test procedure QOS 1600-9 Unit Two was in the RUN mode at a thermal power of 2460 MWt.

VI. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The consequencei of this occurrence were mininal since the reactor buildi' j to suppression chamber vacuum relief system consists of tw 100% flow capacity subsystems. Operation of either subsystem will maintain the pressure differential less than the external design of the primary containment. Redundancy of lir.es Justify reactor operation with one valve out of tertice for repairs, for a period of seven days. The redundant 2-1601-20B valve did operate properly when tested.

Vll. CAUSE:

The 2-1601-20A valve did not open due to the failure of the air line solenoid operated valve 2-1601-50A. When the solenoid coil was de-energized, the solenoid core failed to return to the de-energized position; thus, not allowing air to open the 2-1601-20A valve. This was a mechanical failure of the four-way Versa Model VGS-4422-V-10-31 solenoid valve.

Vill. CORRECTIVE ACTION:

Work request 5276-78 was written for electrical maintenance personnel to investigate the problem. The Versa solenoid valve was replaced, and the valve was again tested to be ope rab le.
