On February 3,1979, while performing surveillance procedure QOS 30-2, RHR Service Water Vault Bed Plate Drain Check, bed plate drain valves A01-4999-57, 58, and 60 did not fully close on their respective float switch signals or on loss of instrument air to the valve operators. As required by Technical Specification 3.5.H., these valves are to close upon the loss of air or power, high level in the equipment drain sump (signaling a possible flood), or high level in the vaults. Work request number 597-79 was written to repair the 1-4999-57, 58 and 60 valves.

There has been one previous occurrence of an RHRS service water pump drain valve failure at Quad-Cities Station documented in R0 50-254/77-16.

VI. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The safety implications of this occurrence are minimized by the fact that each of the subject valves has a check valve located between it and the bed plate drain. This would have prevented any backflow into the RHRS service water pump vaults from outside the vaults or from one vault to another in the event of a flood.

Vll. CAUSE:

Upon investigation, it was discovered that the internal return springs had corroded and failed. The resulting lack of valve plate tension prevented the normally open valves from properly closing and seating.

The A01-4999-57, 58, and 60 valves are manufactured by the Fisher Controls Company and are 2 inch, model number 513RGS.

Vill. CORRECTIVE ACTION:

The corrective action taken was to replace the valve return springs inside drain valves A01-4999-57, 58, and 60. In addition, the other bed plate drain valves A0 1-h999-56 and 59, were inspected and the return springs also replaced as preventive maintenance. The bed plate drain valves were satisfactory retested on February 14, 1979
