On May 24, 1979, while conduc- 1g the suppression chamber to drywell vacuum breaker monthly exercise (Q0S 1600-1), vacuum breaker 1-1601-320 failed to give a closed indication. It coulc not be determined if the valve actually failed to close or if it just was a limit switch problem, because of the inaccessibility of the vacuum breakers. Subsequently, Technical Specification 3.7. A.4.b could not be met. The Unit was in the process of being shut down for a scheduled maintenance outage. Work request 2868-79 was written to investigate the problem. An event of a similar nature was reported in RO 77-49/03L.

VI. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The suppression chamber to drywell vacuum breakers on Unit One had'all been successfully operated previously on April 26, 1979 During the exercise test performed on May 24, all other vacuum breakers operated properly; thus, the safety implications of this occurrence were minimal.

Vll. CAUSE:

An inspeccion of the v&lve indicated that the stem was binding, thereby not allowing the disk to fully shut. During disassembly, a rough spot was found on the bushing at one end of the valve stem.

Vill. CORRECTIVE ACTION:

The defective bushing was replaced as well as the packing on that end of the valve stem. The vacuum breaker was then tested satisfectorily.
