ENCLOSURE 1

NOTICE OF VIOLATION Pennsylvania Power and Light Susquehanna Steam Electric Station Units 1 and 2 Docket Nos.

50-387; 50-388 License Nos.

NPF-14; NPF-22 EA No:

98-140 During an NRC inspection conducted in the period January 20, 1998-March 16, 1998, a violation of NRC requirements was identified.

In acco'rdance with the "General Statement of Policy and Piocedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:

10 CFR 50, Appendix B, Criterion XVI, Corrective Actions, requires, in part, that measures shall be established to assure that conditions adverse to quality, such as failures, deficiencies and nonconformances are promptly identified and corrected.

PPSL Nuclear Department Administrative Procedure, NDAP-QA-702, Condition Report, implements the Appendix B requirements and requires, in part, personnel to identify and report conditions adverse to quality so that appropriate actions can be taken to preclude repetition.

Contrary to the above, on multiple occasions prior to November 25, 1997, the licensee failed to establish adequate measures to assure that a condition adverse to quality involving the standby liquid control (SLC) system accumulators was promptly identified and corrected as evidenced by the following:

Prior to September 1997, on multiple occasions including September 19, 1995 and September 22, 1996, SLC accumulators were found below the pressure required by procedure MT-053-003, SLCS Accumulator Maintenance.

For each event, no Condition Report was initiated and no evaluation was done to justify that the low pressure condition in the

accumulators did not adversely impact the ability of the SLC system to perform its intended safety function.

Following maintenance on both Unit 1 SLC accumulators in September 1997, improper tightening of the cap on the schrader valves on the accumulators caused the valves to leak, depressurizing the accumulators.

This degraded condition was not identified until November 25, 1997, when both Unit 1 SLC accumulators were found below the required pressure.

Pressure is required in the SLC accumulators by plant procedure and provides a measure of protection against the potentially detrimental pressure pulsations produced by the SLC pumps.

The NRC has concluded that information regarding the reason for the violation, the corrective actions taken and planned to correct the violation and prevent recurrence and the date when full compliance willwas achieved is already adequately addressed on the docket in, the letter transmitting this Notice of Violation and in the letter from PPRL dated June 10, 1998.

However, you are required to submit a written statement or explanation pursuant to 10 CFR 2.201 if the description therein does not accurately reflect your corrective actions or your position.

In that case, or if you choose to respond, clearly mark your response as a "Reply to a Notice of Violation," and send it to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I and a copy to the NRC Resident Inspector at the facilitythat. is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
