AppendixA NOTICE OF YIOLATION Pennsylvania Power and Light Susquehanna Units 1 and 2
Docket Nos.
50-387/50-388 License Nos.:
NPF-14/NPF-22 EA 89 208 During the Nuclear Regulatory Commission (NRC) inspection conducted on August 21-25,
- 1989, a violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions",
( 1989),
the following violation was identified.
10 CFR 50.54 (1) states, in part, as a condition of the facility operating
- license, that the licensee shall designate individuals to be responsible for directing the licensed activities of licensed operators who are senior reactor operators licensed pursuant to 10 CFR 55.
- states, in part, that the licensed individual must pass an annual requalification operating test.
Contrary to the above, a senior reactor operator, who had failed an annual requalification operating test administered on January 11,
- 1989, was designated by the licensee to be responsible for directing the licensed activities of licensed operators on fourteen (14) eight hour shifts from January 18 to January 24, 1989, for Unit 1 and from January 27 to February 2,
1989 for Unit 2. This individual was retrained and retested success-fully by the licensee on February 24, 1989
'his violation has been evaluated as a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power and Light Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply including:
( 1) the corrective action steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid, further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending this response time.
