APPENDIX A NOTICE OF VIOLATION Pennsylvania Power & Light Company Susquehanna Unit 1 Docket No. 50-387 License No. NPF-14 As a result of the inspection conducted on August 7-8,
- 1989, and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C (Enforcement Policy) (1988), the following violation was identified:

10 CFR Appendix B Criterion IX requires that measures be established to assure that special processes such as nondestructive testing are accomplished using procedures in accordance with applicable codes.
Contrary to the above, the licensee's procedures were riot adequate to prevent restart of the Unit 1 reactor o.". June 30, 1989 with potentially unacceptable ASME Section XI ultrasonic indicav.'ions in the reactor vessel.
These indications had not received appropriate evaluation and review to ensure that the required ASME Code Section XI criteria were satisfied.
This is a Severity Level IV violation.
Pursuant to the provisions of 10 CFR 2.201, Pennsylvania Power 5 Light Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Mhere good cause is shown, consideration will be given to extending this response time.
