The inspector discussed his recent assignment as principal inspector for St. Lucie 2.
3.
Licensee Action on Previous Inspection Findings (Closed) Deficiency (389/80-01-01):
QA indoctrination for manual employees.
The licensee's letter of response dated February 14, 1980 has been reviewed and determined to be acceptable by Region II.
The inspector held discussions with responsible personnel and examined the corrective actions as stated in the letter of response.
Following the recent labor disturbance shutdown, site management at its own initiative has incorporated in the site orientation program, summarizing statements regarding many of the essential features of a
QA indoctrination program.
Manual employees returning to work after the shutdown attended this site orientation.
Attendance at these sessions was mandatory and recorded.
A draft of the necessary changes to revise procedure SQP"19, "Site Quality Procedure - Indoctrination and Training", was reviewed and found to be adequate.
Unresolved Items Unresolved items were not identified during, this inspectio.
(Closed) Inspector Follow-up Item (389/79-19-01):
Construction Housekeeping and Rigging Practices.
The inspector reviewed the licensee's procedure (SQP-47) for pipe erection and observed construction activities underway in the reactor containment and auxiliary buildings. It was evident that debris had been removed, rigging was being accomplished from the ceiling and structural beams, and construction tooling and material disorder were negligible.

Independent Inspection r
a.
Construction Progress Work is continuing on foundations for steam generators 2A and 2B.
Installation of main steam/boiler feed restraints is in process and setting of the nuclear steam supply system by Bigge Power Constructors is scheduled to commence in mid June 1980.
Construction of Unit 2 is currently 38/ complete.
During this inspection, the inspector observed construction activities underway in the containment building, reactor auxiliary building and the component cooling equipment building.
b.
gC Inspector Strike On January 10, 1980, Region II was notified that site QC inspectors employed by the U. S. Testing Company were on strike and supervisory personnel were being used to replace the striking inspectors.
On January 17-18, 1980, a Region II inspector examined the certification records for those U. S.
Testing Company employees who were being used to replace the inspectors on strike.
This unannounced inspection is recorded in report 50-389/80-02.
On January 18, following the IE inspector's departure, the licensee shut the job down starting with the evening shift.
This was done as a precautionary measure due to harrassment of replacement QC personnel.
On Friday, February 1, an agreement was reached at U.S. Testing and on Monday, February 4,, work was resumed at the site.
During this inspection, it was determined that two additional personnel had been hired for NDE level 1 and
activities.
They are currently undergoing training and have not been certified as of April 3, 1980.
No items of noncompliance or deviations were identified.

Principal Inspector Turnover
Site management has been advised that principal inspector responsibilities have been transferred from T. E. Burdette to R. D. Bradley.
The inspector reviewed the site organization chart and held discussions with appropriate management personnel regarding open items and status of constructio.

(Closed) Item 389/79-18-04:
Damage Resulting From Collapse of Guyed Derrick Crane During Hurricane David.
The inspector reviewed the licensee's supplemental response dated
=January 17, 1980 regarding the concerns expressed by Region II related to re-erection of the crane.
On January 29, Region II advised the licensee that based on their response, the Region no longer had any concerns regarding re-erection.
The. licensee's final report on this matter was forwarded to RII on February 1,
1980.
During this inspection the report and associated drawings were evaluated, the erected crane was observed, and discussions were held with the site civil QA engineer.
The inspector raised questions concerning repairs to reinforcement steel in these safety-related structures.
Nonconformance report 830C, design change notification 550-203, 513.609, 550-207, and field change request 2-1379 were reviewed.
Those bars that were only slightly bent were restraightened by cold bending as authorized by architect-engineer Ebasco.
All other damaged rebar was mechanically spliced or lap spliced.
The licensee's final report described the damage sustained to the containment shield wall and reactor auxiliary building, and identified the damage assessment techniques utilized to determine the extent of damage.
Prior to.any repair work or new construction in the affected areas, each area was specifically evaluated for structural soundness and repair measures approved.
The reactor auxiliary building and reactor building shield wall have been restored to original structural soundness.

b.
(Open) Item 389/80-03-01:
Defective Hydramotor Actuators.
On February 21, 1980, the licensee notified RII that six hydramotor actuators, received in safety-related damper assemblies, have been identified as containing springs manufactured from an incorrect material.
The deficient spring (ITT part number 17443A) was identified to NRC and the damper manufacturer by ITT General Controls of Glendale, California under
CFR 21.
The inspector reviewed the licensee's interim report dated March 29, 1980 and discussed the deficiency with the site electrical QA engineer..
This item will remain open pending receipt of the licensee's final report and review by RII.

(Open) Item 389/80-03-02:
Defective Stud Welds.
MJe The licensee on March 20, 1980 reported that eight door frames for the diesel generator building were found to have defective stud welds during installation.
The inspector reviewed nonconformance report number 1071C and associated deficiency reports'regarding the defective welds.
Rejection was based on bend test failures attributed to lack
of 360 degree flash, blowouts at side of stud, and shrink fissures.
This item will remain open pending receipt and review of the licensee's final report.
d.
(Open) Item 389/80-01-03:
Conduit End Bells The inspector examined examples of deep stop and shallow stop end bells installed on PVC conduit.
Discussions were held with the gA electrical engineer and a representative from office engineering.
The licensee's final report is in preparation and is scheduled for submittal on April 14, 1980.

Deficient Pipe Spool Pieces On January 25, 1980, the licensee provided an interim report on the deficiency.
This item will remain open pending receipt and review of the licensee's final report which is scheduled for submittal by May 1, 1980.

Inspection and Enforcement Bulletins (IEB)

IEB 79-02, Revision 2:
Pipe Support Base Plate Designs Using Concrete Expansion Anchor Bolts.
b.
On January 14, 1980, FPM. provided their response to this bulletin revision. It was stated that Unit 2 does not have any expansion anchor applications in concrete block walls or any application utilizing structural steel shapes rather than base plates.
With regard to verification of anchor bolt pre-load, pre-load is demonstrated by means of torque testing and tension testing.
All installed anchors are inspected by torque testing and one out of fiftyare subjected to tensioner testing.
This bulletin will remain open pending further review by RII.

IEB 79-25:
Failures of Westinghouse BFD Relays In Safety-Related Systems The licensee's response dated March 28, 1980 stated that the subject Westinghouse BFD/NBFD relays are not used or planned for use in safety-related systems in Unit 2.

IEB 79-28:
Possible Malfunction of NAMCO Model EA180 Limit Switches at Elevated Temperatures.
Tht licensee reported on February 7 and April 4, 1980 that Namco model EA180 limit, switches have been identified as being installed in certain valves and in back draft dampers for the containment fan coolers.
However, only the back draft dampers were found to contain models dated between 02-79 through 09-79.
The top gaskets of these limit switches have already been replaced by the manufacture.
Inspection and Enforcement Circulars (IEC) and Information Notices (IEIN)
The inspector held discussions with responsible site personnel to ensure that the following circulars and information notices were received, reviewed, and appropriate action was being taken:
