Unresolved Item 50-389/79-06-Ol-RT Technique of B. F.

Shaw Fabricated Welds.

This item concerned the fact that film side pene-trameters appeared to have been used for radiography (RT) of welds where the welds were accessible for use of source side penetrameters.

The piping fabricator has performed RT on a variety of weld sizes to show that RT using film side penetrameters provides equivalent sensi-tivity to RT using source side penetrameters.

The fabricator has also redefined

"accessibility" for use of source side penetrameters to include welds one pipe diameter or less from the spool end for piping 12-inches through 22-inches and greater and all welds in piping 24-inches or greater.

The licensee has not reviewed the above actions for acceptability and therefore, this item will remain open until this is accomplished and until further inspection can be conducted to determine whether Code (ASME,Section III, para. IX-3335.2, 1971 Edition, addenda through Summer 1973) requirements have been violate.

- Control of Welding Electrodes.

The FPSL letter of response dated November 1,

1979, has been reviewed and determined to be acceptable by NRC:RII.

Corrective actions have included destruction of the affected welding rod, retraining of personnel and QA surveillance of field areas.

The inspector held discussions with the welding superintendent and examined the co~e~ct've actions stated above.

These actions are considered adequate.

- Argon Gas Certifications.

This item concerned the fact that individual batch certifications for Argon Gas used in welding could not be located during a previous inspection.

It was verified that individual batch certification is not required and that FPSL considers that a letter from its supplier, which was available, indicating generic compliance with purity require-ments is acceptable certification.

Therefore, no item of noncompliance exists and this item is closed.

No items of noncompliance or deviations were identified.

Unresolved Items Unresolved items were not identified during this inspection.

Independent Inspection Effort The inspector conducted a general inspection of the reactor building and auxiliary building observing activities such as welding rod control, material handling and housekeeping for conformance to site procedures.

Specific activities reviewed included fitting up of weld nos.

2SI-0422-004 and 2PD-0005-012, grinding for weld repair of weld no. 2SI-0113-002A and welding of lugs on spool no. I-12-SI-406.

No items of noncompliance or deviations were identified.

Radiographic Inspection a

This inspection was conducted because concerns were expressed by an individual to NRC:RII that during a recent strike by QC inspectors, radiographs were not being "read" by Ievel II certified individuals as required at St. Lucie, Unit 2.

Site requirements are contained in Procedure QP-2.9, Rev.

1 "Qualifica-tion of QA Audit and QC Inspection Personnel";

Procedure QI-2.7, Rev.

4 "NDE Personnel Qualification and Certification" and Procedure QI-9.3, Rev.

"Radiographic Inspection".

These procedures allow limited evaluation, to specific dimensional requirements only~ by a Level I certified individual and allow evaluation and require-interpretation (equatable to the term "reading" for radiographic (RT) inspection)

by a Level II certified individual.

Only one Ievel II individual is required to review the film and sign the RT repor The inspector reviewed "Radiographic Test and Interpretation Reports" for 42 welds which had.been accepted during January, 1980 which included the strike period.

Many of the radiographs had been signed by the Authorized Nuclear Inspector during January but had been performed and evaluated by site personnel prior to the strike period.

For those radiographs that were interpreted during the strike period, a certified evel TI xndi~ida51 1IKQ~MWhe-Reports.Site-personnel indicated that although this individual was only certified to Level II requirements for work performed for FPSL at St.

tucie, he was certified to the higher Level III requirements by his parent company (United States Testing).

The inspector verified that this individual was present during the strike by reviewing the daily time sheets.

The inspector also reviewed RT film for eight of the 42 welds for conformance to Code (ASME,Section III, 77S77)

requirements.

Magnetic Particle Inspection This inspection was conducted because concerns were expressed to NRC:RII regarding magnetic particle inspections performed at St. Iucie, Unit 2.

The inspector interviewed two individuals certified to Level II require-ments for magnetic particle inspection.

The inspector also reviewed certi-fication records for four inspectors.

Further inspections willbe conducted concerning this item at a later date.

No items of noncompliance or deviations were identified.

Safety-Related Piping - Observation of Welding Activities Installation of piping is being accomplished in accordance with the ASME Boiler and Pressure Vessel Code,Section III, 1977 Edition plus addenda through the summer of 1977.

The inspector observed welding of Weld Nos.

2SI-0173-002, 2CS-0009-002 and 2CH-0938-009 for conformance to procedure and code requirements.

Areas examined included weld history records; weld identification; availability of required documents at the welding site; adherence to welding procedure variables; welding technique including sequence; availability of certification of base metals and filler materials; joint geometry and cleanliness; adherence to fit up tolerances; purging; use of electrode as specified; shielding gas and flow rate control; equipment condition; control of interpass temperature; interpass cleaning and identification of welders.

Safety-Related Piping - Welder Qualification Welder qualification is performed in accordance with the ASME Boiler and Pressure Vessel Code, Section ZI, latest edition.

The inspector reviewed welder qualification records for the welders of record for the welders of record for the weld numbers listed in Paragraph 8 for conformance to site and Code requirements.
