Exit Interview The inspection scope and findings were summarized on September 6,

1979 with those persons indicated in Paragraph 1 above.

Licensee Action on Previous Inspection Findings (Closed) Unresolved Item 389/79-10-01:

Documentation for barrier dam com-paction piles.

The inspector examined the following EBASCO reports in the Unit 2 quality records storage area.

a.

Soils Foundation of the Emergency Barrier Wall, May 1976.

b.

Compaction Pile Soil Stabilization Program, January 1976.

Based on review of the above reports, it appears the compaction piles were installed as stipulated in NRC requirements, Unit 2 PSAR Appendix 2G, EBASCO Specification FL0-9560-69, and drawing number 877-G-715.

This item is closed.

Unusual Occurrence - Inspector Dispactched To Site At approximately 12:00 a.m.

on September 3,

1979, during hurricane David, the boom of the CBRI construction guy derrick crane fell onto a portion of

the Unit 2 reactor auxiliary building.

A preliminary survey of the damage resulting from the falling boom and stabilizing guy wires is summarized below:

'I a.

Transmission lines from Unit 1 Start-up transformed were damaged when

, southwestern guy wire fell across them.

b.

Unit 2 Reactor containment building (RCB) - Construction of the rein-forced concrete biological shield wall of the RCB has.been completed up to the spring line.

Approximately 18 dowels were bent by the northern guy wire when the boom fell.

There are also some minor surface scars on surface of RCB shield wall, but none deep enough to expose rebar.

Reactor auxiliary building (RAB) elevation 82 - The forms for the roof of the NW corner of the RAB had been placed along north to south column lines 2 RA 1 to 2 RA 2 and east to west column lines RAJ to RAL.

This area, the roof of the Unit 2 control room, is approximately 27 x 55 feet in size.

A portion of the crane boom fell on and pene-trated the formed area of the roof and destroyed or damaged approxi-mately half of the forms and form supports.

d.

RAB elevation 62 - The portion of the boom which fell on the formed area of the control room roof penetrated the control room slab between column line RAK and RAL, from column line 2 RA 1 to 2 RA 2 and broke the NS beam in quadrant RAK, RAL, 2 RA 1, and 2 RA 2. This portion of the boom is resting on the broken beam.

The size of the area damaged is approximately 18 by 27 feet.

RAB elevation 43 - East of column Line RAI, the elevation 62 floor slab and roof have not yet been placed.

A portion of the crane foom fell on the elevation 43 slab in this area.

There is no apparent damage to the floor slab, however the columns on column line RAI at 2 RA 2 and on column line RAI at 2 RA 3 were damaged (cracked through).

Three pieces of 4 KV switch gear and one 4160 step down transformer which were installed in this area were destroyed by the falling boom.

RAB walls above elevation 19.5 - A portion of the crane boom fell on the walls which will support the elevation 43 slab in the northeast portion of the RAB (elevation 43 slab not yet placed in this area).

The boom damaged dowels on top of the northeast corner of the north exterior wall and on top of five interior walls.

The boom penetrated the top of interior wall number 2 and caused localized concrete fail-ure in an approximate 4 x 6 foot area.

The concrete in the northern exterior wall was approximately 7 days of age on September 3,

1979.

The licensee is currently formulating plans to remove the pieces of the fallen crane boom from the structure.

After the crane boom and other debris is removed, a thorough field survey will be made to determine the extend of the damage to the RAB and RCB.

Unless other-wise noted above, all concrete in the area damaged by the falling boom

was more than 28 days of age when the boom fell.

The licensee reported this item as a potential 50.55e item to Region II on September 4,

1979.
