Areas Inspected: Special announced inspection to gather information related to an allegation received by NRC on July 9, 1987, that a shift supervisor had been seen sleeping while on shift and that management was aware of the situation and took no action to correct it. .The inspector met with licensee representatives who provided all known information related to the allegation received by NRC that a shift supervisor had been seen sleeping while on shift and that management was aware of the situation and took no action to correct i The following information was reviewed:

matter is -listed in Attachment 1 of this report, Allegation and Review Chronolog .0 Discovery of Sleeping Areas The inspectors reviewed the following information related to the discov-ery on July 7, 1987 of a makeshift bed in the turbine building: July 7, 1987 Memorandum to file documenting the discovery of a sleeping area in the turbine buildin . Management Tour Reports Notes taken by licensee management of inspections perfort - the turbine building subsequent to the July 7, 1987 discover . TMI-2 Night Order Book The information was reviewed in order to understand the relationship between the allegation received by NRC and the discovery of make shift bedding. The result of the inspectors' review on this matter is listed in Attachment 1 of this report, Allegation and Review Chronology.

In addition, management tour reports for the past three months were reviewed to determine the scope and depth of the tours performed by licensee managers. The inspector determined that the tours primarily concentrated on work activities related to operations, maintenance, and radiological controls associated with cleanup of the facility. Emphasis of the tours was not directed towards remote areas of the turbine build- ,

ing where the makeshift bed and other apparent rest areas were foun '

3.0 Security Review of Access Control Records The inspectors reviewed the access control records for the individual named in the allegation for the following periods: November 29 and 30, 1986; December 1 through 5, 1986; April 3 through 7, 1987; June 18

'through 30, 1987; and' July 1 through 4, 1987. These periods encompassed the times.during which the inspectors understood that the individual was alleged to.have been sleeping or inattentive while on duty. The purpose of the review was to determine whether'any abnormal activities could be discerned from the individual's movements during his shifts. The access control records reviewed were, thus far, inconclusive for that purpos However, these records and the'results of the inspectors' review will be further evaluated by NRC Region I personne .0 . Security Personnel Interviews The inspectors interviewed a total of 13 se:urity officers to determine whether personnel had ever observed' sleeping in the plant during their patrol duties. ' A total of seven security officers stated that they ha in. fact observed contractors sleeping,on occasion in the contractors'

lunchroom, electricians shop, and on the roof of what is called the Defueling Test Assembly-(DTA) module. These facilities are all located in the turbine building. In pursuing this matter, the inspectors were informed that most of the sleeping was observed during what was believed

to.be contractor lunch and break periods and that sometimes foremen were present. The security officers assumed the above rest periods were allowed in that they appeared to be scheduled breaks in the presence of supervisio The security officers stated that they had not seen any licensee person-nel sleeping during their duty patrols of the plant. These patrols currently require a control room walk-through at frequent intervals. No control room personnel were ever observed to be sleeping by the security guard .0 Licensee Investigation The inspectors were informed during the conduct of the inspection that the licensee. had hired an investigator, Edward Stier, a New Jersey attorney, to conduct an investigation into the sleeping allegations. Through an informal interview tne inspectors learned that Mr. Stier was tasked to provide the licensee with the following: a determination whether the alleged shift supervisor had in fact been sleeping on dut . a determination on licensee managements' handling of the alle-gation The inspectors also learned that the scope of the investigation would be increased by the licensee should other relevant information be discovered during the conduct of the investigatio .0 Exit Interview The inspectors met with the following licensee representative; on July 11, 1987 to review the information obtained by the inspection team as a-result of the inspection:

'"You would think'that the big shake up at Peach Bottom would really wake (Shift Supervisor) up, but instead of sleeping in his ' office he now disappears in the plant for hours and know one can find him when he is neede If he hasn't changed by now he never will. This is your last chance to do sonething about it before he takes you with hi People are going to be very upset if this comes to light and they find out how l long you've known about this problem and we t' kept begging you to do something about i _

4/9/87 Plant Operations Manager interviews cach member of the Shift Supervisor's crew individually. Notes were taken but believed to be destroye l l

4/10/87 Plant Operations Manager (POM) issues two-page report to file with cc to the Manager of Plant Operations. Report says "most" shift members would swear-that the Shift Supervisor was asleep sometime in the pas (However,POMnow[7/10/87]saysthatreportisincorrect in that most crew members couldn't swear to it when pressed).

4/10 or Site Operations Director briefs TMI-2 Director (Vice President, ,

11/87 GPUN). TMI-2 Director now (7/11/87) indicates he understood at the time that the allegation could not be confirmed or denied. The TMI-2 Director was not shown the two-page memo describing the results of interview ;

6/24/87 Site Operations Director (new Director since 4/10/87) notifies Manager of Plant Operations of an allegation made by the Fuel Handling Shift Supervisor to the Manager, Safety Review Group, that the Shift Supervisor had been sleeping on duty. The fuel Handling Shift Supervisor received this allegation from an I&C technicia l I&C technician claims he saw the Shift Supervisor with newspapers and dozing in the I&C sho (Field notes exist for Fuel Handling Shift Supervisor and the I&C technician, taken by the Site ,

Operations Director).

Site Operations Director concludes from subsequent interviews that the above incident pre dates the 4/10/87 review already documented by the Plant Operations Manage /25/87 Site Operations Director confers with THI-2 Director. Conclusion i

reached is that this incident is related to past accusations and is considered closed. The Site Operations Director terminates his I

During an interview of a TMI 2 manager 'on 6/29/87 by the Director o the TM1-2 Program Office, inquiring on the quality of perfonnance of licensed individuals, the NRC was .infonned of the earlier allegation

about it 3 months ago and he turned it over to  !

I told him'to inform you of this problem or I woul It seems i nothing has. changed since then and now it is time to let the news media in on this cover u I know of 7 people on (the Shift Supervisor's) shift that will swear under oath that they have seen him sleeping on more than one occasion. Ask (Manager of Plant Operations) and (Site Operations Direc-tor) about this cover up. All their names might soon be in the news print - will yours?"

This same allegation was received by GPUN Public Relations, THI-2 Site Director, and GPUN Presiden NRC Region I was informed by the TMI-2 resident shortly after the receipt of the allegation in the TMI-2 Project Offic In the evening, GPUN President contacts Ed Stier, (a N.J.
