Andersen, Resident Inspector, Region III 2.0 Background On July 9, 1987, the NRC received an anonymous allegation that a shift supervisor had been sleeping while on shift, and that members of licensee management were aware of this issue and had failed to correct it. A special team inspection was conducted on July 10-11, 1987, to determine the circumstances associated with the allegation and other issues related to this matter. (SeeInspectionReport 50-320/87-07). As a result of

! the NRC review of information provided by General Public Utilities Nuclear Corporation (GPUN), the team determined that four other related allegations were received by GPUN since December 198 In addition, as reported to the NRC on July 9,1987, a number of sleeping / eating / play areas were discovered by the licensee in the TMI-2 turbine building since July 7, 1987. The licensee agreed to meet with the NRC to discuss its i

understanding of these and other related issues.

3.0 Summary of Presentation

- William T Russell, Regional Administrator, welcomed the representatives of GPUN and stated the purpose and scope of the meeting. The purpose was to discuss GPUN's knowledge of allegations of a shift supervisor sleeping while on shift, the discovery of sleeping areas inside the turbine building, and the issue of how company management handled these matters. Of concern was the apparent delay in notification of the NRC of earlier allegations received by company management concerning the shift superviso P. R. Clark, GPUN President and Chief Executive Officer, began the discussion for the utility stating that company representatives were here to provide a status report on the above concerns. In particular, both units at TMI would be included in the discussion The licensee presented the chronology of events related to the sleeping allegations dating back to the initial allegation received by a licensee manager in December 198 The NRC asked why GPUN thought other individuals at TMI were not involved in allegations of sleeping while on watc I

- The licensee stated that the allegations thus far point only to the l individual in question and that the sleeping areas found in the I turbine building do not appear to be related to the alleged shift i supervisor. The licensee also stated that if others individuals were fcund to be involved, their investigation into this matter would be expande I

- The licensee discussed the discovery of a bed and other sleeping / eating / play areas in the TMI-2 turbine building since July 7, 1987. Initial corrective actions included dismantling the areas and increased surveillance of those areas by THI-2 manager Licensee management is considering closing off unused areas of TMI-2 l to reduce accessibility to remote areas. The licensee also discussed the company policy relative to sanctions for both contractors and GPUN employees concerning sleeping on sit !

- The NRC requested that GPUN provide the results of their ,

l investigation into this matter, to NRC's Office of Investigations as they are developed. The licensees investigation is being conducted by Edward Stier, a New Jersey Attorney. The licensee agreed to keep the NRC informed during the investigation and restated the purpose of their investigation. The Stier investigation is to: determine whether the alleged shift supervisor was in fact l sleeping or otherwise inattentive on duty.

- The NRC emphasized that.the quality and depth of Stier's 1 investigation is important. The licensee agreed to cooperate with j'

the NRC throughout the investigationLand also to provide the. final report when availabl The licensee provided reasons why it is unlikely that sleeping has l

occurred at TM1-1. The licensee explained why the nature of the operations being conducted, a GPUN policy on sleeping, off-shift l

'

management tours, and increased oversight by the Quality Assurance Department on crew alertness, all assure that attention is focused against sleeping. In addition, the Vice President of Unit 1 personally talked with each shift supervisor concerning the Peach Bottom incident to ensura alertness is emphasized by shift member Unit 1 managers also schedule maintenance and surveillance work during backshif ts to keep crew members active and meet with the -

off-going 11:00 p.m. - 7:00 p.m. shift to discuss the activities conducted that shif The NRC stated that further inspection would be conducted by the Unit I residents to ensure sleeping related issues at Unit 2 are not associated with TM1- The NRC asked GPUN to discuss company policy regarding informing NRC about allegations received by the licensee. The licensee discussed its policy on informing NRC of allegations and stated that in this particular case, NRC should have been informed earlier of the reported allegations of sleeping by a shift superviso The Regional Administrator concluded the meeting stating that NRC is concerned about the allegations and is interested in receiving accurate facts on this matter. He additionally stated that the information requested by the NRC in a letter from him to the utility, for the purpose of fact finding by the NRC, is now considered to have been provide I
