Section Inspection Summary Inspection on October 13-16, 1981 (Report 50-298/81-19)

Areas Inspected:

Routine, unannounced inspection of licensee action on previously identified items and inspection of welding activities related to the Mark I Torus modifications.

The inspection involved 20 inspector-hours by one NRC inspector.

Results:

In the two areas inspected, no violations or deviations were identified in one area. One violation was identified in the other area (violation - failure to follow instructions for welding paragraph 2).

Licensee Action on Previous Inspection Findings (Closed) Unresolved Item (8114-04):

This item was unresolved pending review of the CB&I welding procedure qualification test report to dc vmine if a change in welding technique had been authorized.

The NRC inspector reviewed the welding procedure qualification test report, Production Test Assembly (PTA), Revision 1.

Since the PTA report is silent regarding any deviations from the contract Technical Specification requirements, the change in welding techr.ique from that required in the contract Technical Specification was not authorized.

This item is therefore upgraded to a violation.

Mark I Torus Modifications a.

Review of Procedures The NRC inspector reviewed three CB&I welding procedure specifi-cations (WPSs) and supporting procedure qualification reports (PQRs)

with no discrepancies noted from the requirements of NPPD Contract I

Observation of Work The NRC inspector observed activities related to the torus modifi-cations, specified in NPPD Contract No. 8-28, which included: welding of a vacuum breaker reinforcement plate, piece 12BL on Sketch SK-119; welding of a downcomer brace, piece Mark 44-4 to 44BR on Drawing 43; welding of the high pressure coolant injection (HPCI) turbine exhaust sparger pipe, weld CF on Drawing R3; final visual inspection of HPCI turbine exhaust sparger pipe, welds CA, CB, and CC on Drawing R3; and radiographic examination of sparger pipe weld CA.

In the areas of weld and inspection documentation, weld procedure compliance, welder qualification, electrode control, inspector certi-fication; no discrepancies were noted from the requirements of the following documents:

NPPD Contract No. 80-28, " Mark I Torus Modification" CB&I Nuclear Quality Assurance Manual CB&I Procedure APIN, " Alteration Procedure for Torus Modifications" CB&I Procedure WPS-E7018 #1, " Welding of Carbon Steel Supports" CB&I Procedure WPS-E705-2/E7018, " Welding of Carbon Steel Pipe" CB&I Procedure VT5X, " Visual Inspecticti" CB&I Procedure RT9X, " Radiographic Examination cf Pipe Welds" No violations or deviations were identified.
