Radiation Survey Discrecancies During August 1980 a TMI-2 Radiological Deficiency Report (RDR)
(Number 80-0014) was issued. The report addressed discrepancies in the radiation survey results for materials trcnsferred to Unit 2 by Unit 1 on August 18-19, 1980. The Unit 2 Rad Con foreman, who initiated the RDR, notified Unit 1 on August 20, 1980, that measured radiation levels were higher than those signified on survey tags attacned to materials transferred to Unit 2.

During review of the radiation survey discrepancies, the inspector examined the Unit 2 Rad Con daily logbook and noted that the following was documented:

Rad Con fc.eman that materials had been received on December 18, 1980, and December 19, 1980, which were mislabeled as to radiation levels.
January 5,1980, Unit I was notified that two bags of material

transferred to Unit 2 were being returned due to improper radiation survey. One bag, which was labeled '. n;P/hr by Unit 1, read 800 mR/hr gamma, and greater that. 16 Rad /hr Bcta when surveyed by Unit 2 Rad Con Technicians.
(Unit 2 RDR number 81-0011 was issued on January 5,1981, concerning the incident.)

Cne bag identified as 0.5 mR/hr gama was surveyed by Unit 2 Rad Con Technician and read 50 mR/hr gama, and 1.6 Rad /hr Beta.

Interviews with Unit 2 Personnel

During February and March 1981 the inspector interviewed Unit 2 Rad Con personnel concerning survey techniques.
The inspector reviewed Unit 2 survey records for material transferred from Unit 1 to

From this review, it was determined that in each instance,

appropriate documentation and reporting was taken by Unit 2 personnel.
While the inspector conducted these interviews it was stated that Unit 2 Rad Con personnel had met with Unit 1 Rad Con personnel on '
February 20, 1981, and resurveyed the materials transferred on February 19, 1981, and that a gamma reading in excess of 50 mR/hr on a bag labeled 0.5 mR/hr had been verified by both groups.

Interviews with Unit 1 Personnel Or. February 25, 1981, the inspector interviewed the Unit 1 Rad Con foreman on duty concerning the transfer of radioactive materials to Unit 2.
The apparent radiological survey discrepancies between Unit 1 and Unit 2 were discussed. _In particular, the inspector discussed the transfer of materials to Unit 2 on January 5,1981.
On February 26, 1981, the inspector met with the Unit 1 Deputy Manager Radiation Controls, and informed him of the noted discrepancies of the radiation surveys of materials transferred to Unit 2.
On February 27, 1981, the inspector met with representatives of the Unit _1 Radiological Controls Department to discuss the discrepancies in radiation surveys. At this meeting, the inspector was told that RDR number 81-0011, issued January 6,1981, had not been received by the Unit I action addressee, and that to investigation of the problem had been initiated. The inspector was told that a Unit 1 Radiological Irvestigation Report (RIR) was being filed, and that a complete investigation into the matter would be undertaken.

On March 5,1981, the inspector met with representatives of the Unit 1 Radiological Controls group and ciscussed the craft of the RIR report.
From these discussions and interviews, the inspector determined that although Unit 1 personnel responded to the survey

problem as of February 27, 1981, with an immediate investigation, no previous attempt to evaluate, correct, or document the numerous survey discrepancies had been made.
The failure to evaluate, in a timely manner, the radiation survey discrepancies which were reported during December 1980, January and February 1981 constitutes an apparent item of noncompliance with radiation survey requirements as defined in' 10 CFR 20.201(b) (50-289/81-06-01).

Exit Interview The inspector met with licensee management (denoted in Paragraph 1)
and with representatives of the NRC TMI Program Office at the conclusion of the inspection on March 31, 1981.
The inspector
- summarized the inspection findings.
DisCJssion during the meeting also included a review of portions of the Unit 1 Radiological Investigation Report. NRC representatives requested that in reply to the item of noncompliance, management address and correct,if necessary, Unit i radiation survey procedures in order to avoid similiar survey problems in the future.
