Subject:
Groove Pin Dedication Equipment Affected by this Information Notice: Fisher Controls International LLC 301 South First Ave P.O. Box 190 Marshalltown, Iowa 50158-0190 USA T (641) 754-301 1 F (641) 754-2830 Items subject to this Fisher Information Notice (FIN) were provided to Tennessee Valley Authority (TVA) Watts Bar Nuclear P l ant per TVA Order 702507 (F i sher Order Number 025 -N230331922).
Specifically , this FIN applies to a plug/stem assembly that Fisher Controls International LLC (Fisher) supplied to TVA, item number 15A2560X072 (Equipment).
In th i s Equipment , the essential-to-function groove pin was erroneously provided as commercial-grade.
Purpose: The purpose of th i s FIN i s to alert the customer that , as of 22 January 2016 , Fisher became aware of a situation wh i ch may affect the of the aforemen t ioned Equipmen t, i ncluding i t s safety-related function. Fisher is i nforming the customer of th i s circumstance in accordance with Section 21.21 (b) of 10 CFR 21. Applicability:
This FIN applies only to the Equipment supplied by Fisher t o TVA Watts Bar via the aforementioned order. Discussion:
The supplied Equ i pment i n question features a plug tip which is connected to the plug by a simple threaded j oint. A groove pin is used to prevent the plug t i p from unscrewing from the plug block (please see attached Item 1for part identification). Because the threaded joint is not self-locking , the g r oove pin serves as a primary lock i ng dev i ce. It i s therefore considered essential-to-funct i on and requ i res dedication per FMP2K27 (Control of Commerc i al Grade Items to be Dedicated for Use in Nuclear Safety-Related Applications). However , review of the documentation package reveals that the groove pin was not subjected to the requirements of FMP2K27; thus , the part was supplied as a commercial
-grade item.
