1.0INTRODUCTION

BackroundOnApril29th,1981,theEssexCorporationwascontractedbytheFloridaPowerandLightCompany(FPRL)toprovidehumanengineering(HE)consultingservicestoassistintheassessmentofseveralnuclearpowerplantcontrolrooms.OnetaskofEssex'swastoconductapreliminaryprelicensingreviewoftheSaintLucieunit2controlroom(CR).ThisreportpresentstheresultsofthepreliminaryHEreview.SaintLucieunit2(SL-2)isaCombustionEngineering(CE)PWRwithWestinghouseturbinesandgenerators.TheEBASCOCorporationservesastheArchitect/Engineer.TheplantislocatedbytheAtlanticOcean,approximately12milessouthofFortPierce,Florida.TheplantisatwoloopPWRthatproduces880Megawattofelectricity.Theplantusesseawaterforcooling.AsofMay,1981,plantconstructionwasapproximately70%complete.ThecontrolroomisconfiguredasshowninFigure1.Asshown,themainboardsofthecontrolroom(CR)arearrangedmoreorlessasadoglegwithplantsupportandmonitoringpanelsandboardsplacedatvariouspointsintheroom.Moredetaileddescriptionsofthemainboardsareprovidedhere:oPanel201oPanel202oPanel203oPanel200oPanel205oPanel206Thissectioncontainstheemergencydieselgenerators,mainplantbuses,turbineandgeneratorcontrolsanddisplaysfortheplant.Locatedherearecontrolsandinstrumentsconcerningcondensatesystem,circulatingwater,mainandauxilliaryfeedwatersystemandsteamgenerators.Thispanelcontainsthecontrolsanddisplaysfor,theprimarycorecoolingsystem.ILocatedherearetheprimarynuclearinstrumentationandcontrolelementassemplydrivemechanisms.ThisistheChemicalandVolumeControlSystempanel.ControlsanddisplaysfortheEmergencyCoreCoolingSystemandComponentCoolingWaterSystemarecontainedinthepanel.

I 2e0EVALUATIONAPPROACH,BYTASKS2.1AssembleProectLibrar2.1.1~obectivesoToassemblealibraryofCRrelateddocumentationstosupporttheNTOLreview,oToorganizethedocumentationforeasyaccessbyallNTOLreviewteammembers.2.1.2MethodoThefollowingdocumentswereacquiredforinclusioninthelibrary:Systemdescriptions(FSAR)PRIDsSoftwaredescriptionsTrainingmaterialsPanellayoutdrawings/CRfloorplanAnnunciatorresponseproceduresInstrumentlists2.1.3DataReductionandAnalsisNone.2.2DeveloPhotoMosaic2.2.1~obectivesoTophotographtheCRpanelsforassemblyintoaI/OscalemosaicofthecurrentCRconfiguration,oToprepareaphotomosaicsuitableforpanelevaluationanddevelopmentoftypicalreports.

I 2.2.2MethodCRpanelswerefilmedincolorinuniformsections(samesizedphotorepresentationofpanelareas).Eachframewasdevelopedintoa1/0scaleoforiginal.Photoswerethenpiecedtogetherintoamosaicforeachmajorpanelsection,pastedtoaboard,andcoveredwithanacetatematerial.2.2.3DataReductionandAnalsisNone.Themosaicwasusedfor:1)surveys(selected);2)checklists(selected);3)examinationofcontrol/displaylayout;0)HEDreviewsbyEssex;and5)developmentoftypicalbackfits.2.3ConductofSurvesControlroomsurveysarepresentedinthissection.Theseinclude:1.Lighting,2.Emergencygear,3.Noise,0.Genericchecklistapplications.GiventhattheSt.Lucie2CRhasnotyetbeencompleted,lighting,emergencygear,andnoisesurveydatahavebeentakenfromSt.Lucie1.Totheextentthatthesedatageneralizetothesisterplant,St.Lucie2,theymaybeusedfordiscrepancygeneration.SpecificationsfortheabovewerealsosuppliedtoFP*L,sinceunit1datawillnotbepreciselycoincidentwithunit2.Wherediscrepancies'wereidentifiedinunit1,thisisindicatedintheResultssectionofthisreport.Objectives,instrumentation,methods,datareductionandanalysis,andfindings(whereapplicable)foreachsurveyisdescribedbelow.2.3.1LihtinSurve2.3.1.I~ObectivesTheobjectiveofthisevaluationwastomeasuretheambientilluminationinthecontrolroomandtoassessitsimpactontheoperators'bilitytoreadandinterpretdisplays,controls,labeling,andprintedmattersuchasdrawingsandprocedures.

IIII 2.3.1.2InstrumentationTheambientilluminationwasmeasuredusingaTechtronicsJ-16photometerandprobe,calibratedpriortouse.2.3.1.3MethodTheambientlightingsurveywasconductedundernormallightingconditions.Measurementofambientilluminationwastakenat15operatorpositions.Thesepositionsincludedthefollowing:oReactorOperator'sworkdesk,oSeniorReactorOperator'sworkdesk,oEachpanel,oEachpointwherereadingofprintedmaterialmightberequired,oBackpanelsbehindthemaincontrolboards.Themeasurementsweretakenforallpositionsselectedundernormallightingconditions.Thelightmeterwasheldwiththemeterpointedattheceiling,wherethepositionrequiredreadingaspecifictypeofprintedmaterial.Thedatawererecordedonaformforsubsequentanalysis.2.3.1.0DataReductionandAnalsisDatawereplacedontheformshowninAppendixA,andcomparedtotheNUREG1580Guidelinescoveringcontrolroomillumination.2.3.2EmerencGarments2.3.2.i~ObectiveThistaskyieldsinformationneededtocompletethehumanengineeringchecklists.Ingeneraltheresultsindicateproblemsinperformingcontrolroomoperationswhileusingtheprotectiveclothingand/orbreathingapparatus.2.3.2.2MethodTheaccessibilityandusabilityofCRemergencygarmentswasevaluatedbyvisualobservationoftheequipment,andacursoryuseofselecteditems(e.g.,facemasks),byaHEanalyst.

Il~III 2.3.2.3DataReductionandAnalsisAlistwasmadethatdescribesfactorsthatmightreduceoperatorreliability,forinstance:oVisibilityofbreathingapparatusfacemask,oTactilediscriminationthroughgloves,oSpeechimpairmentthroughfaceplate,'Hearingimpairment(noiseofbreathingapparatus),oSizeofglovedhand(inadvertentactuation).2.3.3GenericChecklists2.3.3.1~ObectiveTheobjectiveofthisreviewwastodetermineiftheSL-2controlroommanifestshumanengineeringshortcomingscharacteristicofothernuclearpowerplants.Theobjectiveinreviewingtheissueslistedasgenericindustryproblemswastoprovidethereviewerwith1)apointofreference,and2)abroad,generalreviewofthecontrolroomwithemphasisonidentifyingmajorissueswhicharehighlylikelytooccurbasedonreviewsofsimilarplants.2.3.3.2MethodUsingalistofgenericdiscrepancies,apanelbypanelandsystembysystemreviewwasconducted.NotedwereCRcharacteristicswhichviolatedhumanengineeringpracticeslistedinthegenericchecklist.2.3.3.3DataReductionandAnalsisApplicationofgenericchecklistsimmediatelyidentifieshumanengineeringdiscrepanciesfromtheguidelines.Astheyareidentified,discrepancyreportsarewrittenandenteredintotheHEDreviewprocess.

2.3.0Noise2.3A.i~Ob'ectiveTheobjectiveofthissurveywastomeasuretheambientnoiselevelsinthecontrolroomfromvariousoperatorpositionsandtoassessitsimpactontheoperators'bilitytoverballycommunicateand/ordiscriminateaudiblesignals.ThemeasurementsweremadeusingtheSaintLucieunit1controlroom.2.3.0.2InstrumentationIntheperformanceofthisstudyaGeneralRadioCorporation,Model1983soundlevelmeterwasused.2.3A.3MethodTheperformanceofthisevaluationrequiredtheconsiderationofnotjus'tnormalcontrolroomnoisebutanyfactorsthatcanaddtotheoverallnoiselevel.IncludedinthisweretheoccasionalnoisesofveryShortdurationthatcancausehighpeaklevels.aoNoiseConditions-Thenoisesurveystartedwithabasalnoiselevel.Thisistheambientnoisewithoutalarms,printers,orcommunicationsequipmentcontributing.Oncethismeasurementhasbeentaken,potentialnoisesourceswereintegratedintotheambientenvironment,bymeasurementofpeaknoiselevels.Thefollowingwereconsideredaspotentialnoisesources:oAudiblealarms,oTypersandprinters,oCommunicationsequipment(ringingtelephones,P.A.s,radios),oEmergencyoratypicalenvironmentalcontrolsystems(airconditioning,exhaustfans),oLoudconversation,oExternaltotheControlRoom,oOpendoorsleadingoutofthecontrolroom.

IIIIII b.positionsthatrequiredverbalcommunicationand/orauditorydiscriminationofasignal.ThisincludedtheReactorOperator'sdeskposition,theSeniorReactorOperator'sdeskposition,twopointsontheeachpanel/board,andtwopositionsatbackpanelsrequiringcommunication.Twomeasurementsweretakenateachposition,onemeasuringbasalnoiselevels,theothermeasuringpeaknoiseoverapproximatelyatwominuteinterval.Measurementsweretakenflat(dB),andinaweighting(dB"A").AformusedtocollectthosedataiscontainedinAppendixA.Theinstrumentwascalibratedbefore,duringandafterconductofthesurvey.2.3.0.0DataReductionandAnalsis'Thecollecteddata,werecomparedtothe1580guidelines.ValuesthatexceedtheestablishedlimitswerenotedandaHumanEngineeringDiscrepancyReportwascompleted.2.3.5DesinConventionSurve2.3.5.1~Ob'ectiveThissurveyyieldedalistingofdesignconventionsusedinthecontrolroom,andwasusedtoidentifyinterfacesthatviolateCRestablishedconventions.2.3.5.2MethodExamplesofcontrols,displays,labels,etc.,weresurveyedandconventionsrecorded.Wheredesignconventionswereused(mostorallinterfacessurveyedfollowthesameoperationalrules)theconventionwasnoted,anddeviationsfromtheconventionsdocumentedfordiscrepancyreportingpurposes.2.3.5.3DataReductionandAnalsisWheredeviationsfrompredominantCRconventionswerenoted,HumanEngineeringDiscrepancyreportsweregenerated.

I 2.3.6Checklistin2.3.6.j~ob'ectiveTheobjectiveofcompletingthechecklistwastocomparethedetailsofthecontrolroomdesigntotheHumanFactorsGuidelines.Thedetaileditemscontainedinthecheckistsallowedforasamplingevaluationfromthesystem,panelandgenericcomponentlevel.2.3.6.2InstrumentationCertainchecklistitemsrequirethatphysicalmeasurementsbeperformed(tapemeasures,soundmeters,andsoforth).2.3.6.3MethodsThecompletionofthechecklistsrequiredaccesstothecontrolroom,abasicfamiliaritywiththecontrolroomandthesystemsbeingevaluated,andtheassistanceofaqualifiedoperator.Onceappropriatechecklistswereselected,apacketofchecklistswasmadeupforpanelsorsystemstobeevaluated.Thechecklistswerethencompletedinthefollowingmanner.Eachchecklistitemwasconsidered,intheCR.Whentheitemdidnotapply,an"N/A".wasplacedinthecheckcolumn.Iftheitemwascompliedwith,a"yes"wasplacedinthecheckcolumn.Wheretheitemwasnotcompliedwith,a"no"wasplacedinthecheckcolumnandthediscrepancywasdescribedindetailinthenotescolumn.Whereappropriate,examplesofdiscrepantcomponentswerenotedforreportingpurposes.2.3.6.0DataReductionandAnalsisThechecklistswerereviewedfordiscrepancies(itemsmarked"no")andcomparedtotherelevant1580Guidelines.AHumanEngineeringDiscrepancyReportwascompletedforeachitemthatdidnotcomplywiththeguidelines.

ClI 2.3.7ProcedureReviewandDeveloment2.3.7.1~ObectivesTwoobjectivesofthistaskwere:l)toassistFPRLingeneratingprocedures(emergency,startup,andshutdown)usingCEprocedurespecificationsasabaseline,and2)toevaluatetheproceduresaccordingtotheguidelines(format,writingstyle,andsoforth)..3..Essexinvolvementwithproceduredevelopmentinvolvesprovisionofaprocedurespecificationtoaidinthedevelopmentofhumanengineeredprocedures.2.3.8HEDReviews/HEDreviewsentailtwoactivities:preparationofhumanengineeringdiscrepancyreports(HEDs)andreviewprocessing.2.3.8.1PrearationofHumanEnineerinDiscreancReortsPersonnelinvolvedintheDataCollectiontask(3e0)wereinstructedtocompleteaHumanEngineeringDiscrepancy(HED)formforeachandeveryidentifiedincidencewherethecontrolroomdesigndoesnotcomplywithHumanEngineeringGuidelines.Noattemptwasmadeduringactualdatacollectiontodeterminewhetherornotaparticulardiscrepancywassufficientlyimportanttoreport.HEDswerecompletedforalldiscrepanciesincludingenvironmental,layout,instrumentation,jobdesign,etc.2.3.8.2~ObectiveTheobjectiveofthistaskwastoprovidecompleteandaccuratedocumentationofallhumanengineeringdiscrepanciesinthecontrolroom;toanticipatethespecifichumanerrorsthatmightresultfromthediscrepancy;toreporttheguidelinesviolated;andwherenecessary,torecordthelikelyresponseoftheplantsystemtotheerror.

2.3.8.3MethodTheSL-2controlroomwasreviewedbydatacollectionpersonnelandoperator-controlroominterfacesthatdidnotmeettheHumanEngineeringGuidelineswereidentified.Thesediscrepancieswererecordedonaform,"HumanEngineeringDiscrepancyReport"(seeAppendixA),whichincluded:aeb.Ced.e.Ashorttitleforthediscrepancy;Hardwareorproceduresitems,nomenclature(label)andpanellocations;Humanengineeringguidelineswhichwereviolated;Operatorerror(s)thatmightresultfromthediscrepancy;Asrequired,plantandsystemlevelconsequencesoftheseerrors.Wherethereareanumberofinterfaceswiththesamediscrepancy,thesameproceduresinvolved,andthesameconsequencesofoperatorerror,one"generic"HEDwasgenerated.ThefinalstepinHEDpreparationwastheidentificationofsuitablebackfitstoreduce/eliminatethediscrepancy.Mostdiscrepancieswereconsideredcorrectablebyanyofseveralbackfitswithdifferentpotentialsforreducingoperatorerrorlikelihoodanddifferentcosts.Forinstance:oChangeofinstrumentationtypeorlocationoAdditionofrepeatingdisplaystoimprovecontrol/displayrelationshipoDemarcationlinestoimproveoperatorlocalizationofcontrolsand/ordisplaysoUseofswitchguardstoreducethelikelihoodofinadvertentoraccidentaloperationoUseofalarmsorwarningstoadviseofapotentialerroroUseofswitchordisplaycolorcodingtoimproveoperatorlocalizationoUseofdisplayrangemarkings(e.g.,normal,emergency)toimprovedisplaydiscriminabilityatadistanceoUseofmimiclinestoimprovesequentialcontrol/displayoperations12

3.TheRM-11systemusesagriddisplay(schematiclayout)torepresentmonitorlocationswithintheplant.Agriddisplaylogolocatedinthelowerrightcornerofthescreenwillflashwhenanalarmconditionisfound,directingtheoperatortothecorrectgrid.ThisgridlogolacksspecificgridnumbersbutresemblesthelocationoftheGridSelectionbuttons.Labelingoffunctionandselectionbuttonsisnotthebest,butcouldbesufficientwithpropertraining.0.TheLampTestbuttononlylightsthosebuttonsthathaveanon/offfunctions;however,astheseappeartobetheonlysimpleindicatorlights(orbacklightpushbuttons)onthekeyboardthisappearstobeadequate.5.Thedisplayterminalhasa"HEALTH"labelnearthe"POWER"labelandisreportedbyGAtorepresentpowertotheterminal.However,theyarenotentirelysureofthis.Ifthisistrue,thelabelshouldbechangedto"POWERON".6.The"ENTER"functionbuttonislocatedontheleftsideofthenumerickeyboard,thesecondinacolumnofidenticallyappearingbuttonsandcouldcausesomedelayinoperating.Thisisnotatimecriticalsystem;however,betterlocationofthe"ENTER"buttoncouldimproveoperatorperformance.7.Thebrightnesscontrolislocatedrightonthefrontforindividualoperatoruse,withtheothercontrolbuttons(Vertical/Horizontalhold,etc.)locatedbehindalockedpanelontheCRT.8.ThereisalsoaDegaussingbuttonlocatedonthefrontpanel.GAsaystheCRTneedstobedegaussedatintervals.Withalltheautomationofthissystem,itseemslogicalthatthisfunctioncouldalsobeautomatic,otherwiseaPreventativeMaintenanceprogramneedstobeincludedforit.9.The"LIT"buttonconvertsallfunctionkeystoalphabeticalkeysforsystemprogramming.ThiscapabilityimposestherequirementforanumberofbuttonsthatarenotusedbyControlRoomOperators.Thiscouldcausesomeconfusion;however,thisisnotatimecriticalsystem.Itisrecommendedthatthislabelbechangedtoonemoredescriptiveofitsfunctionsuchas"NOR/ALPHA"fornormaloralphabetic.Additionally,allfunctionkeysshouldcarryadoublelabelindicatingfunction(undernormalconditions)andthealphacharacter(whenprogrammingfunctionison).

IIIII RM-23CABINETMODULESANDRECORDERS1.TheRM-23display/controlmodulesaredigitaldisplayunitsthatarebackupunitsforsafetysystemmonitorsonly.ThisisduetotheCRTnotbeingseismicallyqualified.TheRM-23'swillprimarilybeusedbymaintenancepersonnelorintheeventofanaccidentthatincludesfailureoftheCRTterminal.RM-23'swillrequireextensiveoperatortrainingasallfailureindicationsareinnumericalcode.2.The"ON","ALERT"and"HIGHALARM"lightsareverysmallandnotveryintense.Thismakesthemdifficulttodetectundernormalilluminanceconditions.Theseshouldbeenlargedandtheirintensityincreasedifthereisanypossibilitythattheymustfunctionasprimarydisplays(i.e.,primaryradiationannunciators).3.ThereareanunequalnumberofstripchartrecorderscomparedtotheRM-23modules.DifferentcombinationsofreadingsfromRM-23modules(upto3)willbedisplayedontheserecorders.Therationaleforthisisunclearatthistime.0.Thestripchartrecordersmustbereadupclose.Beyondfourfeetanobservercannotdetermineiftheyarestandardscaleorlogscale.5.Thereisnoalarmmechanismtonotifytheoperatorofstripchartfailure,paperrunningout,orexhaustedinksupply.6.Thestripchartrecorderscanbefullyremovedfromthecabinetoncetheretainersareloose.Locksshouldbeinstalledsuchthattheycannotbeinadvertentlypulledallthewayoutanddroppedwhenchanginginkandpaper.7.Thepen/channeltocolorontheseLeedsandNorthruprecordersaredifferentfromthemajorityfoundinNPPCRs.Baseduponapreviousrecommendation,itissuggestedthatthesequencebechangedtothefollowing:Pen/ChannelColorRedGreenBlue IIIIIIII:

S.ThestatusindicatorlightslocatedontheRM-23cabinetsbetweenthemodulesandrecordersarereportedbyGAtoindicatewhichRM-23moduleisnotinservice.ThefuelhandlingareamonitorshaveredundantRM-23's,onesetinUnit1andtheotherinUnit0.,Thisoption,andthereasonfortheredundancyisunclearatthistime.Also,thelocationoftheswitchfordeterminingUnit1orUnit0operationisnotknown.Itispossiblethattheselights,whenlightedwillindicateadeactivatedunit.Thiscouldbeconfusingasitestablishesalighted=offandunlighted=onrelationship.Alternativestothistyperelationshipshouldbeseriouslyconsidered.9.ItshouldberequiredthatnoRM-23modulesbemountedsuchthatthereadoutandassociated"HIGH"and"ALERT"lampsareabove70inchesorbelow01inchesfromthestandingsurface.Ifanysuchmodulesaretobeusedasprimarydisplayswhichmustbereadpreciselyandfrequently,thesemodulesshouldbemountedsuchthattheirdisplaysarenothigherthan65inchesorlowerthan50inchesfromthestandingsurface.

I RECOMMENDATIONSBasedontheaboveresults,theGADigitalProcessRadiationMonitoringSystemappearstohavesomehumanengineeringdesignproblems.Theaboveevaluationisbasedonaverypreliminaryandcursoryreviewandshouldnotbeconsidereddefinitive.TheCRTdisplaysandtheinteractivesequencescouldnotbeadequatelyevaluated.TherecommendationofEssexisthattheconfiguration,onceoperational,beevaluatedindetail.
