Unit ted'1 DDPS SOFTWARE ERROR Vendor designed software change for Beacon was based on latest copy maintained by
- vendor The new scaling constants were not permanently saved into the master program or sent to software vendor A revision of the software was installed that did not include the new FW flow constants
$ 994 PWO changed span of FW flowtransmitters from 0 - 790.6" to 0- 800" New FW flowscaling constant values were entered into DDPS constant locations PCIM 4029-196 provided the design for Installation of Beacon system and its connection to DDPS The 1994 changes to the feedwater flow'constants were overwritten 6-10-94 3-8-96
Uriit41 DDPS SOFTWARE ERROR The Beacon PC/M scope was limited to the hardware and DlD NOT include the necessary changes to the DDPS software he PC/M DID NOT provide post modification testing requirements for the DDPS software CM determined that the F flowscaling constants were for the old 0-790.6" transmitter span.
The software V8Vspecified by the software vendor challenged the Beacon outputs but failed to check other aspects of the DDPS software such as the secondary plant calorimetric.
Reactor startup for Unit¹1 Cycle ¹14 During repairs to a FW flow transmitter, iCM found that DDPS calculated reactor power level was lower than actual power by 0.63%.
ERT Long Term Corrective Action:
Develop generic V8V requirements to challenge all critical attributes within both UnitN and Unit¹2DDPS for all software changes.
7-25-96 2-21-97 3-5-97
Unit P2 DDPS SOFTWARE ERROR.
PC/M referenced generic V &V plan developed as corrective action to Unit
¹1 event. Overlap between portions of test plan was inadequate.
PC/M scope DID Include software changes Inadequate in-house DDPS software expertise to catch vendor error.
PC/M did not provide adequate post modification testing for DDPS software changes.
PC/M ¹96-147, supplement 1
provided design for rescaling of FW flowtransmitter span from 0 - 800" to 0- 900" DDPS vendor contracted to revise software.
FW flow Believable Value Processor scaling was not revised.
New generic VBVof critical DDPS attributes was performed.
Did not detect Incorrect FW flowscaling constants in Believable Value Processor.
At 80% power ascension plateau DDPS calorimetric was determined to be 5% lower than manual calorimetric (This comparison was a specified part of V 5 V plan) 5-17-97 5-27-97

PMAIPRIORITIZATIONPROGRAM Allopen Pcs to be sorted based on the followingprioritization system:
I.
Musts Includes all PMAIs required for continued plant operation.
Examples include: Written NRC/Regulatory commitments, QA responses, justified personnel safety issues and load threatening/load reduction items.
II.
Hi Wants Includes PMAIs that have a quantifiable regulatory or economic benefit.
Examples include: PMAIs that impact plant indicators (OPWAs, PWOs > 12 months, old clearances, leak list, etc.), plant availability, plant/component reliabilityor have a high economic payback.
III.
Low Wants/Recommended Cancellation Candidates Includes PMAIs that involve enhancements/betterment.
Examples include: Design enhancements, process improvements or items with low payback.
Includes PMAIs that are no longer an issue, should be canceled based on management directive and are not cost effective.
Note the PMAIclosure/cancellation process shall be reviewed as part ofthis effort to assure PMAIcancellation does not violate any existing plant procedures.
Open PM&database reports have been generated and are currently under Engineering review.
Plan and completion schedule to be reviewed with plant management.

Actions Taken 1)Maintenance has delivered the subject material to NMMfor evaluation, to determine ifthe items should be returned to stock (MRM'd) or placed in investment recovery. 2)Maintenance has changed applicable procedures to require the return of repairable materials to NMMwithin 30 days ofjob completion. 3)NMM has evaluated the subject material and MRM'd where appropriate. Remaining material is in investment recovery. 70 INVENTORYREDUCTION GAME PLAN ~ Reorder Reviews (adjust min/max levels) ~ Vendor Agreements Identify areas were vendors are capable of maintaining inventory in lieu of on site inventory, and establish agreements where possible. ~ Cons umables Current St. Lucie inventory includes approximately one million in consumables that can be expensed and removed from inventory. ~ Inventory Technical Assessment Perform technical review of inventory to determine appropriate stocking levels and needs. This includes over max material. Write offunnecessary material and place in investment recovery. Fasteners Perform technical review of stocked fasteners for consolidation, elimination of redundant, and vendor supplied "standard" fasteners. Standard description will also allow open market biding, resulting in at least a 20% savings per item. PTN/PSL Common Simulator Parts Currently PSL carries all PTN/PSL common simulator parts in the PSL inventory ($417,000). This burden should be shared 50/50 by both sites. Extended Life RCP Seals (N-9000) 1998 PSL carries $900,000 in seal cartridges, to facilitate RCP changes during outages. The N-9000 seal does not require cartridges, and can be rebuilt during outages like PTN.
