1.0 INTRODUCTION

The intent of this lesson is to introduce / review the Serious Control Room Fire Abnormal Procedure. The material (s) that you are responsible for is/are stated in the objective for this lesson.

2.0 PRESENTATION A. Symptom (s) that indicate entrance into this abnormal procedure:

1. Extremely serious fire in the Control Room or Obi #1 Cable Spread Room requiring a prolonged evacuation of the Control Room.

NOTE: The fire is assumed to be intelligent and may have the capacity to place control circuits is the least desirable position.

CAUTION: The required operator actions to mitigate Obj #5 this event involve disabling many plant safety systems to prevent inadvertent operation caused by the fire. The procedure assumes the opera-tor actions will place the plant in the safest possible condition and then isolate the compo-nent in that position by removing its power or failing its operating device. The operator actions in some cases are in violation of various technical specifications, however, these actions are necessary to mitigate the event to stabilize the plant in Hot Standby.

1. Reactor Operators Obi #3 and #7
a. Trip the reactor and turbine.

c. Control OTSG 1evels with the auxiliary feedwater pumps.

e. Mocitor pressurizer level - control at greater than 100 inches. EO-1 throttles MU-211 to maintain level. If makeup pumps are not available use Piggyback Mode to control RCS invento ry.
f. Control RCS pressure as necessary by having the ASS nonitor the gauge at RC-54.

NOTE: Auxiliary Shutdown Panel pressure indications may not be functioning if the SFAS cabinets have de-energized.

Perform couparison checks to verify the indicators; if they are correct use the indicators periodically verify the indicators,

4. Reactor Operator 1 -
a. Obtain flashlight, radio and tool kit from the cabinet and proceed to the locations which ATT 3 directs.
b. At the Diesel Generator Rooms (1) Fails open the component cooling water outlet valves. CC1474 & 1471.

(2) Turn isochronous droop switch to isochronous.

(3) Turn voltage regulator droop switch to 4

off.

J (4) Adjust frequency to 60 Hz.

(5) Adjust voltage to 4300 VAC.

(6) Place all emergency control transfer  !

10. Shift Technical Advisor
a. Call Emergency Preparedness Supervisor to Obi (6 declare a Site Emergency and have all support personnel assemble.
b. Take the following reference material to the ASP.

(1) Mechanical P & ID's.

(2) Electrical P & ID's.

(3) Valve Location Book.

(4) Load Distribution Book.

(5) PP and EP/AB Procedure Books.

C. Initial and Final Plant Conditions

1. The plant is in Mode I cr 2 at the start.
2. The plant is taken to Mode 3 when the procedure Obj #2 is followed. Guidance is provided for Cooldewn to ,

Cold Shutdown, i

3. RCS heat removal is via the steam generators, Obi d4 auxiliary feedwater is supplying the SG's the AVV's are naintaining SG pressure, (TSAT-RCS terp)

Natural circulation is in progress.

4. Condition of selected plant systems,
s. Electrical Distribution '

(1) Non-essential 13.8 KV, 4.16 KV, and TP-2, TP-3 480 V buses have de-energized, The diesel generators are supplying the essential 4.16 KV and 480 V buses. The control power knife switches have been opened to A, B, C2 and D2 buses to prevent inadvertent operation of the breakers. All emergency control power transfer switches on C1, D1, El, and F1 have been placed in " LOCAL".

b. Service Water (1) The Service Water System has been TP-4 aligned to supply the essential CCW, CTMT air coolers, and ECCS room coolers. The return line has been lined up to the
  • forebay. Service water has been isolated to AFP suctions, H2 dilution blowers, and TPCW heat exchangers. Power has been <,

removed from all necessary motor operated valves to prevent inadvertent operation.

c. Component Cooling Water f (1) The CCW System, is aligned with both TP-5 essential headers in service and side I also' supplying the non-essential lines.

The CCW to makeup pump is handjacked open.

The CTMT isolations have been de-energized open to provide RCP thermal barrier ani letdown coolers cooling, ,CCW to the DHR coolers is ensured by failing open CC-1407 and CC-1469 open. Power has been removed from all necessary motor operated valves to prevent inadvertent operation.

Auxiliary Feedwater '

(1) The AFW System is in a normal SFRCS trip TP-6 lineup with each pump taking suction from the CST and feeding its own OTSG. Power has been removed from the isolation valves to prevent inadvertent operation. The pump speed is being controlled in manual from the Auxiliary Shutdown Panel. Care must be taken to transfer suction to service water when the CST is emptied.

e. Main Steam (1) The MSIV's have been closed by the SFRCS trip and are maintained closed by isolat-ing instrument air and nitrogen. The atmospheric vents will be used to control' OTSG pressure in manual control from their' handwheel.

-f. Makeup / Letdown /RCS (1) The makeup pumps are aligned to take TP-7, TP-8, TP-9 suction from the BWST and discharge via the bypass to MU 32 into the RCS. The mini-recirc is realigned to the BWST to prevent filling the makeup tank. Seal injection has been isolated along with seal return. All demineralized water sources to the makeup system are isolated.

Power leads from the control room switches to the oil pumps have been cut to prevent loss of the oil pumps. RCS precsure is being monitored by a gage on the sample line from the pressurized. Letdown is isolated at MU 2B and MU 2A. The PORV has been isolated by closing the block valve.

Power has been removed from all necessary motor operated valves to prevent inadver-tent operation. These lineups will maintain the plant in a stable Hot Standby post trip condition until directed to cooldown when offsite support personnel arrive. The lineups assume no active failure of a component occurred prior to a preventive action being taken by the operator. If a failure has occurred, the operator must take appropriate corrective actions to restore the required lineup.

If the fire adversely affects the Makeup Pumps, such that RCS makeup via the Makeup System is not available. The LPI and HPI pumps may be "Piggvbacked" as outlined in Attachment 52 of AB 1203.02, " Serious Station Fire." Prior to starting the LPI pumps mini-recire, flow must be ensured by verifying CCW is available to the decay heat coolers.
