4.3 BLOOD ALCOHOL CONCENTRATION (BAC) - Blood alcohol concentration is a measure of the mass of alcohol in a +

volume of blood such that an individual with 100 mg of alcohol per 100 .nl of blood has a BAC of 0.10 percent.

Blood alcohol concentration can be measured directly from blood alcoholorinderived from a measure of the concentration of breath specimen.

CHAIN-OF-CUSTODY - procedures to account for the integrity i of each speciman by tracking specimen handling and storage from the point of collection to final disposition.

4.5 CCLLECTION SITE - A place designated by Company Management where individuals present themselves for the purpose of providing a specimen of urine, breath, or blood to be analyzed for the presence of drugs tr alcohol.

4.6 COLLECTION SITE PERSONNEL - personnel who instruct and assist individuals at a collection Lite and who receive 3nd make an initial examination of the specimen (s) provided by those individuals.

J 4.7 COMPANY PERSONNEL - Those personnel assigned to work at Davis -Besse who are employed by Centerior Energy Corporatica or its subsidiaries.

4.8 CONFIRMATORY TEST - A second analytical procedure to identify the presence of a specific drug or drug metabolite which is independent of the initial screening test and which uses a different technique and chemical principle from that of the initial test in order to ensure reliability and accuracy. (At this time Gas Chromatography / Mass Spectrometry (GC/MS) is the onl method for cocaine, marijuana, opiates,y authorized confirmatory 1

amphetamines, phencyclidine). For dctermining blood alcohol levels, a confirmatory test means a second test using another breath  ;

i alcohol analysis device. Further confirmation upon demand j shall be by Gas Chromatography analysis of blood.

i 4.9 CONFIRMED POSITIVE TEST - The result of a confirmatory test that has established the presence of drugs, drug ,

I metabolites, or alcohol in a specimen at or above the cut-off level, and that has been deemed positive by the Medical Review Officer (MRO) after evaluation. A confirmed positive i test for alcohol can also be obtained as a result of a t confirmation analysis without of MRO bloodevaluation.

4.14 FITNESS FOR DUTY -(FFD) - Reasonable assurance that an individual will perform his or her duties in a reliable and i trustworthy manner. t 4.15 FOLLOW-UP TEST - Chemical testing at unannounced intervals, to ensure that an individual is maintaining abstinence from drug and/or alcohol abuse.

DAVIS BESSE ADMINCTRATIVE PROCEDURE PAGE PAoCEDutt NUMBE A l AEvisioN Drug cnd Alcohol Tcating Praccoo 7 01 IS-AC-00018 4.16 DEPARTMENT OF HEALTH AND HUMAN SERVICES (DHHS)/ NATIONAL i INSTITUTE ON DRUG ABUSE (NIDA) CERTIFIED LABORATORY - A urine and blood testing laboratory that maintains certification by the National Institute on Drug Abuse to perform drug testing under the Department of Health and Human Services.

4.17 ILLEGAL DRUGS - Those drugs included in schedules I througn V of the Controlled Substance Act, but not when used 4 pursuant to a valid prescription or when used as otherwise authorized by law.

4.18 INITIAL PRESUMPTIVE POSITIVE TEST - The results of a confirmatory test that has established the presence of illegal drugs which have not been reviewed by the MRO.

4.19 MEDICAL REVIEW OFFICER (MRO) - A licensed physician responsible for receiving laboratory results generated by the DBNPS drug testing program who has knowledge of substance abuse disorders and has appropriate medical training to interpret and evaluate an individual's positive i

test result together with his or her medical history and any other relevant biomedical information.

4.20 PERMANENT RECORD BOOK - A permanent bound book in which identifying data on each specimen collected at a collection site are permanently recorded in the sequence collected.

4.21 PRIVACY - Allowing an individual privacy when providing a urine specimen, unless reason to believe exists that the individual may alter or substitute the specimen.

4.22 RANDOM TEST - A system of unannounced chemical tests administered in a random manner to a group so that each person within that group hr.s an equal probability of selection.

4.23 REASON TO BELIEVE - Beljef that a particular individual may alter or substitute the urine specimen.

4.24 SPLIT SAMPLE - A portion of a urine specimen that may be stored by the Health Center to be tested in the event of I appeal.

1 4.25 SUITABLE I!iQUIRY - A best-effort verification through a maximum 5 year history, but not less than 3 years, to ascertain if a person was, in the past, testod positive for illegal drugs, subject to an Employee Assistance Program (EAP) (except self-referral), or made ineligible for activities, or denied unescorted access to a nuclear power plant in accordance with a Fitness for Duty (FFD) policy.

l Infornation concerning the drug and alcohol I testing ia confidantial and snall not be l disclosed to persons other than the subject l individual, his/her representative, or to those l personnel who have a need to have access to the l information to perforn assigned duties. No l l individual authorized to have access to this l infotmation may disc 1cse this information to l personnel who do not have a need to know. l 6.1 General _Reauirements 6.1.1 All urine specimens and blood samples shall be tested by laboratories certified by the National Institute on Drug Abuse (NIDA).

6.1.2 Specimens collected as required by this procedure ,

shall not be used to conduct any other analysis or j test without the permission of the tested i individual.

6.1.3 During pre-access, post accident, periodic physicals, for-cause, random, and follow-up drug testing, urine specimens shall be tested fort

a. Cocaine
b. Marijuana
c. opiates  !
d. Amphetamines ,
e. Phencyclidine.

6.1.4 For-causa testing or analysis of any urine specimen suspected of being adulterated or diluted through  ;

l hydration or other means may be tested for any illegal drug.

6.1.5 Supervisors, co-workers, and relatives of the individual being tested shall not perform any collection, assessment, or evaluation procedures.

b. The performance of drug and alcohol testing in accordance with this procedure.
1. Proficiency. in the performance of drug and alcohol testing shall be documented as part of the Personnel Training Package.

a. Maintaining the integrity of the specimen collection.and transfer process,
b. Assuring the tested individual's privacy and  ;

modesty.

c. Avoiding any conduct or remarks which may be construed as accusatorial or inappropriate.

6.2.3 The Medical Review Officer shall ensure his alternate receives training in the following documents:

b. Ensure there is no other source of water in the enclosure where urination occurs.

6.1.4 If the Health Center is unavailable for use as a collection site and a drug test is required, Health Canter Personnel shc.1 use an on-site or off-site l restroom as follows:

a. Secure and post the restroom against access to >

other than those associated with the tenting l process.

b. If possible, place a toilet bluing agent in the toilet bowl and any accessible toilet tank.  ;
c. If no bluing agent is available to deter specimen dilution, instruct the individual '

being tested not to flush the toilet until the j specimen is collected.

6.10 Review of_Results 6.10.1 The Medical Review Officer shall:

6.12.2 When notified by the MRO of unsatisfactory performance testing by the laboratory, the ,

i Supervisor - Access Control shall notify the Manager-Industrial Security and initiate an i investigation.  !

6.12.3 Based on the findings of the investigation, the laboratory shall take action to correct the cause of the unsatisfactory performance testing.

6.12.4 The investigation shall document the findings and corrective action taken by the laboratory.

6.12.5 The individual responsible for the day-to-day management of the DHHS certified laboratory shall j sign and date the investigative and corrective action report, j i

6.12.6 Supervisor - Access Control shall forward the report to Nuclear Licensing.

! 6.12.7 Nuclear Licensing shall process the report I according to NG-NL-00807, Regulatory Reports and forward to the NRC within 30 days of the signature l by the management of the DHHS certified laboratory.
