NOTICE OF VIOLATION Cecronwealth Edison Company Docket No. 50-195 Docket No. 50-304 As a result of the inspection conducted on October 27, November 6,11,12, 14 and 20, 1986, and February 17, 1987, and in accordance with 10 CFR Part I, Ippendix C - General Statement of Pclicy and Procedure for NRC Enforcement Actions (1985), the following violaticns were identified:

1. 10 CFR 50, Appendix B, Criterion Y, as implen.ented by the Coninonwealth Edison Quality Assurance Prograr. requires that activities affecting quality be prescribed by docurer;ted instructions, procedures or drawin55 cf a type appropriate to the circumstances and that those activities be accomplished in accordance with the instructions, procedures or drawings.
a. Contrary to the above, repairs made to Unit 1 Diesel IB were accomplished with inappropriate work instructions .that contributed to inadequate descripticr.s cf certain werk functions, confusion, and missing of Quality Control hold points.
b. Contrary to the above, sorw. Cnit 1 Diesel IB work packages did r.ct

' contain " sign offs" by the saintenance mechanic as required by the instructions attached to the Nuclear Work Request.

This is a Severity Level IV vic.lation (Supplement I).

2. 10 CFR 50, Appendix B, Criterior X, as implemented by the Corrnonwealtt Edison QA program, requires that a program for inspection of activities affecting quality be establishec and executed to verify conforinance with docurrented instructions, procedt.res and drawings for accomplishing the activity. Examinations, measurerents or tests shall be performed as necessary to assure quality and where "nold points" are assigned, work will not proceed past the desigrated " hold points" without proper reler.se.
a. Centrary to the above, work proceeded past 13 Quality Control hold points that were not propedy released for inspection, as documer.ted l in five Nuclear Work Recuest packages for Unit 1 IB Diesel.

b. Contrary to the above, Unit 11B Diesel was released for testir.s l without verification that ir.spection was completed and acceptable l

for seven maintenance related Nuclear Work Requests.

Notice of Violation 2 Pursuant to the prcvisicns of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1)correctiveaction taken and the results achieved; (2) corrective action to be taken to avoid further violations; ard (3) the date when full compliance will be achieved.
