APPENDIX A NOTICE OF VIOLATION Yankee Atomic Electric Company Docket No. 50-29 Yankee Nuclear Power Station License No. DPR-3 During an NRC inspection conducted on April 10, 1989 - May 21, 1989, certain violations of NRC requirements were identified. In accordance with the " Gene-ral Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C, 53 Federal Register 40019 (October 13,1988), the violation is listed below:

A. Technical Specification 6.8.1 requires written procedures be established, implemented and maintained that meet or exceed the requirements of Appendix A of Regulatory Guide 1.33, Revision 2 and Sections 5.2-5.2.9 and 5.3 of ANSI N18.7-1972. ,

Section 8 of Appendix A to Regulatory Guide 1.33 requires that procedures be written to implement surveillance of main steam isolation valves and fire protection systems.Section V 5.2.2 of ANSI N18.7-1972, entitled

" Procedural Adherence" requires that procedures be followed, and the requirements for use of procedure be prescribed in writing.

1. Procedure 0P-4644, " Functional Test of the Fire Detection Instrumentation", requires, in step C.V.5, the previously removed or " lifted" electrical lead be restored or "relanded" to its criginal circuit configuration. Contrary to the above, on March 7, 1988 the licensee failed to properly restore the circuitry configuration of a module in the control room main fire panel during surveillance testing, such that the control room emergency air cleaning system was rendered inoperable until the wiring discrepancy was identified and corrected on August 16, 1988.
2. Procedure OP-4661, " Functional Test of the Non Return Valve Automatic Control System", requires that system restorational step 2.1 be performed prior to returning the NRV control switch to the

" Auto" position. Contrary to the above, on April 6,1989 the licensee failed to perform the restorative procedural step during furctioncl testing of the non-return valve (NRV) control system logic which resulted in the inadvertent closure of the No. 1 NRV and required c manual reactor scram to be initiated.

This is a Severity Level IV Violation (Supplement I).

This violation has been categorized by severity level in accordance with the

" General Statement of Policy and Procedure for Enforcement Actions", 10 CFR Part 2, Appendix C. Because the licensee previously implemented corrective actions which achieved full compliance and because corrective measures to pre-vent recurrence were both timely and extensive, no response to the violation is required.
