l ENCLOSURE I DESCRIPTIN: OF VIOL \TIONS Yankee Atonic E1cetric Company Dockat No. 50-29 License 50. D?R-3 Certain activities under your lic2nse appear to be in violation of AEC requirements. These violations are considered to be of Category II and Category'III severity.

Categorv II Violaeions

1. Lriterion V, Appendix 3, 10 CFR 50 states in part: "Activitics affectin; quality shall be. . . accomplished in accordance with these (prescribed) instructions, pre:cdures, or drawings."

t Contrary to the above, maintenance adjustment to Sesnic Class I pipe hangers uas not conducted eith an approved procedure as required by Administrative Procedure AP-0214.

2. Critorion V, appendin B,10 CFR 50 states in part: "...Instructicns, procedures or dra"ings shall include appropriate quantitative or quali-tative acceptan:c criteria for determinin; that important activitics have been satisfactorily accomplished."

Contrary to the abeve, modification maintent,ce to a main coolant check valve was perfor cd althout acce?tance as required by Administrative Procedure AP-0214 bein; specified.

3. Technical Specifications, Paragraph B.2, which incorporatos Section 106:2 of the Final Hazards Summary Report states, in reference to .

shutdoun chenistry condition of the main coolant system, " Chloride... l 1ess than 0.1 ppm."

Contrary to the above, chloride concentrations in the main coolant water reached 0.1 ppm on July 2,1974. It is noted this item eas l

reported to the Regional Director, Regulatory Operations, Region I, I in your Iceter dated July 11, 1974 So respo.1c to this violation is required.

Cateentv III Violations

1. Criterion VI, Appendi:: B, 10 CFR 50 states in part: "... Changes to documents chcil be revicteed and approved by the same organizations that p:rformed the original review and approval..."

Contrary to the above, corrective maintenance was performed on a main coolant pump uith a procedure containing unauthorized changes contrary to the requirceents of Administrative Procedure AP-0001 concerning procedure changes.

2. Criterion V, Appendix B, 10 CFR 50 states in part: " Activities affecting quality shall be... accomplished in accordance uith those (prescribed) ins tructions, procedures , or drawings."

Contrary to the cbove, shift supervisors and centrol room operators f ailed to docu::.s c.t their revicu of active Bypass of Safety Functions and Jumper Control Requests as required by Administrative Procedure AP-0018.
