This refers to the routine resident safety ir.:pection conducted by Messrs.
J. Macdonald and M. Markley of this office on April 10 - May 21,1989 at the
i
Yankee Nuclear Power Station, Rowe, Massachusetts.
The inspection consisted of
observation of activities, interviews with personnel and document reviews.
The
results of the inspection are described in the NRC Region I Inspection Report
enclosed with this letter and were discussed with Mr. N. St. Laurent of your
staff at the conclusion of the inspection.
Based on the results of this inspection, it appears that certain of your acti-
!
vities were not conducted in full compliance with NRC requirements, as set
forth in the Notice of. Violation, enclosed herewith as Appeudix A.
The viola-
tion concerns two examples of previously unresolved issues regarding the fail-
ure to properly perform surveillance procedures as required in Technical Speci-
fication 6.8.1.
In the first example, an electrical lead was relanded at the
wrong circuit location following surveillance testing which rendered the con-
trol room emergency air cleaning system inoperable for approximately five
months in 1988 ( March - August).
In the second example, a restorative prn-
cedural step was not performed during surveillance testing which resulted in
the inadvertent closure of a non-return valve and the initiation of a manual reactor scram on April 6, 1989.
Because your previously iniplemented corrective actions to these events have
achieved full compliance and because i.orrective measures to prevent recurrence
were both timely and extensive, i.o response to the violation is required.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice", a copy of
this letter and the enclosures will be placed in the NRC Public Document Room,
