1.0 INTRODUCTION
Wisconsin Electric has conducted a formal Control Room Design Review (CRDR) at the Point Beach Nuclear Plant (PBNP) in accordance with the require-ments of NUREG-0737, Supplement 1. This Summary Report describes the CRDR status, review findings, proposed control room design improvements and schedules for implementation.
1.1 Purpose The primary purpose of the PBNP CRDR was to ensure that the PBNP control room will support operation during postulated accident conditions, in order to meet the requirements of NUREG-0737, Supplement 1.
1.2 Overview of PBNP CRDR

The specific objectives of the PBNP CRDR were as follows:
(1) To identify human engineering discrepancies (HEDs) in the control room.
(2) To assess the significance of identified discrepancies and to establish corrective actions and schedules.
(3) To verify that selected design improvements provide the necessary correction and do not introduce new HEDs.
(4) To coordinate improvements with other emergency response capability improvements.

1.2.2 Scope The detailed scope of the PBNP CRDR is described in Revision 1 of the

" PBNP Program Plan. A flowchart depicting the interaction between the various phases is shown in Figure 1.
1.3 Revision 1 Program Plan Revision 1 to the PBNP Program Plan primarily reflects changes made to Revision 0 as a result of the NRC staff review of Revision 0 (NRC letter to Wisconsin Electric dated January 22, 1985) and the in progress audit conducted by the NRC staff on December 2-6, 1985. The significant differences between the original Program Plan (Rev. 0) dated July 1984 and the revised Program Plan (Rev. 1) dated March 1987 are as follows:
NUREG-0700, Section 6, " Control Room Human Engineering Guideline," was used as the primary control room survey checklist instead of the NUTAC checklist. However, a point-by point comparison between the two docu-ments resulted in the addition of several NUTAC items not covered in O

- In the Assessment Phase, the Human Engineering Discrepancies (HEDs) are now given a 1 through 9 HED Assessment Priority before any cost / benefit analysis is performed. Figure 4-1 of the revised Program Plan shows a flowchart documenting the new process of priority assignment.
The CRDR schedule has been updated to reflect the project extension requested following the NRC's in progress audit. See Figure 5-1 of the revised Program Plan.
The Level of Participation Summary table has been modified per the NRC's review of the initial program plan and to show actual participation. See Table 5-1 of the revised Program Plan.
General Physics Corporation (GPC) was chosen as the human factors con-sultant. This position was unfilled at the time of the initial program plan submittal.

- A procedure specifying the nature of the HED assessment process was included in the Rev. 1 Program Plan (see Appendix J).
- A procedure specifying the nature of the HED resolution process was-included in the Rev. 1 Program Plan (see Appendix K).
Revision 1 of the Program Plan describes the manner in which the Planning, Execution, Assessment, and Documentation Phases of the PBNP CRDR were con-ducted up to this point in time. It also describes how Wisconsin Electric intends to complete the remaining phases of the CRDR effort.
1.4 Emergency Response Capability Integration The PBNP CRDR is the final activity at Point Beach for improving emer-gency response capability as required by Supplement 1 to NUREG-0737.
Activities previously completed include implementation of upgraded Emergency Operating Procedures (EOPs), the addition of a Safety Assessment System (SAS),
the upgrading of post-accident monitoring equipment per Regulatory Guide 1.97 (nearly completed), and the addition of emergency support facilities. The CRDR helps ensure that these other activities were properly integrated in order that the PBNP control room can support operation during postulated acci-dents. Although none of the 812 HEDs identified during the CRDR were judged significant enough to prevent mitigation of postulated accidents, Wisconsin Electric is committed to correcting the majority of these HEDs to further enhance our emergency response capability.
1.5 E0P Velidation The Execution Phase task of validating control room functions was expanded to provide for the final validation of the upgraded Point Beach E0Ps, which are based on Revision 1 of the Westinghouse Owners' Group (WOG) Emergency Response Guidelines (ERGS). The upgraded E0Ps underwent extensive verification by both the Point Beach Manager's Supervisory Staff and an independent veri-fication team and also underwent an initial validation prior to implementation.
This initial validation consisted of validation of the Revision 0 and Revision 1 WOG ERGS on the Callaway and Seabrook simulators, respectively, and validation of the Point Beach-specific E0Ps on the Kewaunee simulator. The CRDR validation
, i was expanded to include walkthroughs on the Point Beach full-scale, photographic mock-up of fourteen accident scenarios plus all remaining E0P procedural steps q .
C not addressed by the scenarios. Although a number of HEDs were identified l during this validation, no significant problems with the E0Ps were found. The resolution of these HEDs during the CRDR Correction Phase will complete the E0P validation effort.
2.0 CRDR STATUS The following summarizes the status of each CRDR Phase as outlined in the Program Plan (Rev.1):
2.1 Plannino Phase The P8NP CRDR Planning Phase consisted of five major tasks. Those tasks were development of a Program Plan, selection of CRDR team members, selection of a human factors consultant (HFC), human factors orientation for the CRDR team as well as plant orientation of the HFC personnel, and procurement of a O full-scale, photographic mock-up to be used during the CRDR.
2.1.1 Program Plan Revision 0 of the PBNP CRDR Program Plan was submitted to the NRC for review in July 1984. NRC review comments were received, and a meeting was held with the NRC in April 1985 to discuss this Program Plan evaluation. The results of this review were positive and the NRC concluded that if the CRDR was completed in a manner consistent with the description presented in the Program Plan with certain recommended changes, it should meet the requirements of NUREG-0737, Supplement 1. Revision 1 of the Program Plan reflects the changes recommended by the NRC, actual implementation details of the CRDR, and inclusion of comments received during the NRC audit of December 1985.
2.1.2 Selection of CRDR Team Members The review team included personnel with expertise in the fields of human O factors engineering, plant operations (i.e., licensed operator), nuclear i
i engineering, instrumentation and controls engineering, post-accident monitor-ing instrumentation, environmental qualification, Safety Parameter Display Systems (SPDS) and Emergency Operating Procedures. The team was supplemented l
by personnel from other disciplines such as nuclear, mechanical, electrical and civil engineering, when required. Resumes of key members of the review ll team are included in Revision 1 of the Program Plan. The CRDR organization is-l 1
shown in Figure 2. '

i 2.1.3 Human Factors Consultant i
The CRDR Team included two qualified human factors consultant personnel provided by General Physics Corporation. Other human factors specialists and support personnel were also provided by General Physics to support the PBNP l CRDR effort-(see resumes in Appendix L of Program Plan, Rev. 1).
2.1.4 Review Team Orientation j
The review team was assembled at PBNP for a two-day orientation. The i
orientation included a one-day course in applied human factors presented by General Physics' human factors consultants. The purpose of the course was to provide PBNP team members with an understanding of human factors methods f and techniques. The second day of the orientation focused on the CRDR ;
approach for PBNP. An orientation and tour of the Point Beach plant and control room was included for those team members unfamiliar with Point Beach.
l At the end of the orientation, tasks were specified and the project schedule f reviewed so that each individual understood his role on the review team. 1 1
Two CRDR team members were replaced during the effort. Mr. Edmond Mercier from Nuclear Plant Engineering replaced Mr. Gary Sherwood, after Mr. Sherwood '

- was reassigned to another gro.p. This replacement occurred early in the pro-f gram and Mr. Mercier attended a human factors course similar to that given
, other CRDR team members. Mr. Mark Keehan from the Nuclear Systems Engineering I
and Analysis Section replaced Mr. Dennis Blakely, after the HED assessment was essentially complete. Mr. Keehan is scheduled for a human factors course in
! the near future. The balance of the members, however, remained the same, thus f providing the continuity required for this effort, consistent with the Program Plan commitment.

a 2.1.5 Full-scale Photographic Mock-up General Physics Corporation designed and built a full-scale photographic mock-up of the PBNP (Units 1 & 2) control room. This mock-up was used during the control room survey and during the verification and validation tasks of the CRDR Execution Phase. Any changes made to the control room after the photos were taken were surveyed individually in the control room. The mock-up was also used for the initial verification of HED resolutions and is intended to be used extensively in the detailed design and final verification of resolutions.
2.2 Execution Phase 4
The PBNP CRDR review procedures were based primarily on guidance provided in NUREG-0700. Six tasks were performed to identify human engineering discrepancies:

4 1 2.3 Assessment Phase The CRDR Team assessed identified HEDs and recommended actions for their resolution through an iterative review process shown in the CRDR Activity Flowchart (Figure 1). The procedures used for HED assessment and resolution I are contained in Appendices J and K, respectively, of the Program Plan (Rev. 1).

2.4 Documentation Phase A large number of documents were referenced and produced during the CRDR.
At the conclusion of each Execution Phase task, a task report was generated.
All project documentation was compiled and initially stored in a controlled file at General Physics Corporation, Columbia, Maryland. This documentation is being transferred to and will be retained by WE in the future and will be available for NRC audit.
2.5 Correction Phase Plant modifications, procedure revisions, or training required to resolve significant HEDs will be implemented through existing PBNP administra-tive procedures. These procedures address modifications, maintenance work,
) procedure changes, and training. The use of existing administrative proce-j dures will control the extent and means by which plant operators will be made i aware of impending changes to and the use of the modified control panels, systems, or procedures. This also ensures that the potential safety impact of these changes are properly evaluated prior to implementation.
2.6 Effectiveness Phase In order to ensure human factors considerations for control room changes implemented as a result of the CRDR and after the CRDR is completed, a PBNP l Human Factors Standards Document is being written to allow review of such
! changes during design and implementation. To evaluate the human factors j acceptability of all proposed control room modifications, the standard (cur-i rently in draft form) has criteria similar to those used during the CRDR.
Any proposed control room change will be evaluated against the criteria, before the change is implemented.
2.7 Additional CROR Tasks f
' Wisconsin Electric conducted the following two additional CRDR tasks that i
are not specifically called out in NUREG-0700, NUREG-0801, or Supplement 1 to NUREG-0737:

- Safety Assessment System (SAS) location Study
- Reduced Operator Staffing Study O Each of the tasks was conducted in accordance with good human engineering practices (drawing from NUREG standards and principles where appropriate). The procedures for conducting the two special tasks are described in the Program Plan (Rev. 1).
i The SAS Location Study report recommended a location for an additional SAS Cathode Ray Tube (CRT) with PPCS capability in control panels 1-C03 and 2-C03 (the secondary plant control panels). This recommendation is now in the final design stages.
The Reduced Operator Staffing Study report recommended that the current shift manning levels should not be reduced at this time for the condition when 3
one unit is in a cold or refueling shutdown mode. WE is not now considering such a reduction in shift manning levels at Point Beach.
i 3.0 RESULTS/ RESOLUTION
SUMMARY
- The performance of the Execution Phase of the CRDR resulted in the identi-fication of 812 HEDs. None of the identified HEDs were found to prevent accident mitigation. Therefore, none of the proposed corrective actions need
, to be implemented immediately. A number of these HEDs (73) were determined by the CRDR team to be invalid (mis-statements, misinterpretations of actual plant conditions or duplications). These HEDs were " zeroed out" of the HED data base and removed from further consideration by the CRDR Team. Unsigned copies of the remaining 739 valid HEDs are attached. The file copy of each HED, ;

3.2 Assessment /Prioritization Process l
All valid HEDs identified during the Execution Phase were assessed and prioritized according to the significance of each on emergency plant operation.
The HED Assessment Procedure used to assign HED priorities is located in Appendix J of the Program Plan (Rev. 1). Each HED fell into one of 9 HED 1
' assessment priorities based upon the answers to the questions on the HED Assess-ment Flowchart (Figure 3). Priorities 1-3 are accident-related and are the most significant. Priorities 4-6 are Technical Specification-related and priorities 7-9 are the least significant. A numerical listing of HEDs by assessment flowchart priority is provided in Appendix B.
To further assess the significance of each HED, each team member also i
! assigned each a subjective priority ranging from 1 to 9, with I being the most serious. The average subjective priority assigned to each HED by the team is documented on Section II of the HED form. The assessment and subjective prior-ities were then used to help determine whether a particular HED needed to be
! corrected and the nature and schedule for such correction.
3.3 Resolution Process 1
I After all of the identified HEDs were prioritized, the CRDR team developed conceptual resolutions to the HEDs in accordance with the HED Resolution Procedure located in Appendix K of the Program Plan (Rev. 1). The resolu- ,

The conceptual resolutions documented on the HED form have gone through
) an initial verification by mocking up various types of resolutions on the full-j scale photographic mock-up and extensive review and refinement by the CRDR l
i Team. The PBNP Managers Supervisory Staff also reviewed and approved the con- l 4
ceptual resolutions. In addition, selected Duty Shift Superintendents and i
! licensed operators reviewed and commented on the mocked-up resolutions. The final selection of resolutions war also done in consideration of the Human f .

Factors Standards Document being prepared for Point Beach. This Standards Document will be used for the final detailed design of these resolutions.
U 4.0 IMPLEMENTATION t
4.1 Final Verification of Design Improvements Proposed detailed designs of HED resolutions will be drawn in full-size i for new instruments, rearrangements of displays and controls, and certain
, enhancements. These designs will be applied directly to the PBNP control room mock-up, as necessary. A final verification of the resolutions will be per-
) formed prior to implementation on the control boards by conducting a human factors review to ensure that the HEDs are corrected and that no new HEDs are created.
1 j Operating personnel will also review the proposed design improvements, 4 as necessary. If their comments suggest there are deficiencies in the proposed designs, alternative designs may be developed, applied to the mock-up, and reviewed again by operations personnel.
j Where extensive changes of instrumentation or rearrangements of instrumen-i tation are required, a walkthrough by operations personnel will be conducted on E0Ps which exercise the system or control panel which is reconfigured. As l in the original walkthroughs, CRDR team members will observe these walkthroughs and operations personnel will be debriefed following the walkthroughs to ensure that the HEDs are corrected and no new HEDs are created.
1 4.2 Implementation Schedule i
The schedule for implementing HED resolutions is based upon anticipated manpower and other resource commitments; the estimated lead times for design, verification, and material procurement; installation during scheduled outages; and the assessment flowchart and average subjective priorities of the HEDs.
Completion dates are given by resolution category, since the design and pro- !
curement for a given type of resolution will generally be done together. f I
However, it is our intention to address the higher priority HEDs within each j
category first, when that is practical. I

The implementation schedule provided for each category is the current best estimate of the completion date for all resolutions in that category.
Wisconsin Electric is strongly committed to maintaining these schedules.
However, equipment delivery delays, regulatory requirements, plant operational problems, or other unanticipated problems which dilute our manpower and other resources, may cause delays in these schedules beyond our reasonable control.
The following table shows the expected completion dates by resolution category:
