NRC INSPECTION REPORT NO. 50-528/529/530/92-36

This letter refers to the inspection conducted by Messers. D. Acker, C. Clark.-

and H. Freeman of this office during the weeks of October 19 and November 2, "

1992, of activities authorized by NRC License Nos. NPF-41, HPF-51, and NPF-74

and to the dis:ussion of their findings held with members of your staff on

November 6, 1992.

This inspection: '

  • Evaluated the effectiveness of your activities-to assure the operability;

and reliability of check valves in safety-related systems.

  • Evaluated the performance of inservice inspections.

  • Followed up on previously identified items.

Specific areas examined during :this inspection are described in the enclosed

4 inspection report.- Within these areas, the inspection consisted'of selective

examinations of procedures and representative records, and interviews with -

personnel, i

The results of this inspection indicated that you have implemented a-program

to visually inspect check valves and that you were developing the expertise to

monitor check valve conditions-with non-intrusive equipment.- Your program

appeared to be comprehensive and aggressive. However, the inspectors found-

two safety related check valves that were omitted from your LInservice Testing:

Program, errors in your review of check valve generic information, and check

valve program basis documents that had not been updated to reflect current

,

practices. +

Based on the results of the inspection, certain:of your activities appeared to

  • Failure to include two required check valves in the Palo Verde Inservice

Testing Programs, and failure to perform the required inservice testing.

  • Failure to properly document and evaluate nonconforming conditions

identified during a check valve inspection and during a snubber inservice

inspection.

  • Failure to perform check valve examinations required by procedure.

The weaknesses and violations identified during this inspection indicate a

lack of management oversight of the implementation of your check valve

program, particularly assurance of procedure compliance and updates of generic <

information. Both issues have been identified to you in previous NRC

inspection reports. Your recent responses to these problems indicate that you

understand them and that you are taking actions to correct them.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response. In your

response, you should document the specific action taken and any additional

actions you plan to assure that all personnel know and understand your

expectations of procedural compliance. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, Pub. L. No.96-511. In accordance

with 10 CFR 2.790 of the NRC's, " Rules of Practice," a copy of this letter and

enclosure will be placed in the NRC Public Document Room.

Should you have any questions concerning this inspection, we will be pleased

to discuss them with you.
