APPENDIX A

NOTICE OF VIOLATION

Gulf States Utilities Docket: 50-458/85-33

River Bend, Unit 1 Permit: CPPR-145

During an NRC inspection conducted on June 26, 1985, a violation of NRC

requirements was identified. The violation involved the making of changes to

quality assurance records. In accordance with the " General Statement of

Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C

(1985),theviolationislistedbelow:

1. Criterion XVII of Appendix B to 10 CFR Part 50 requires that inspection

records identify the inspector, the results, and the acceptability.

Contrary to the above, the NRC inspector determined that the results for

the general clearances attribute on Inspection Reprt Number P5400381

dated June 12, 1985, had been changed on June 13, 1985, from "not

applicable" to " satisfactory" by a reviewer who neither had performed the

inspection nor had any other inspection documentation to support the

change.

The two supports identified in Inspection Report Number P5400381 were

inspected by the NRC inspector and complied with th? general clearance

requirements of Construction Site Instruction Number 3.1.1, Revision 4.

A preliminary review by the licensee determined ttet e small percentage

of other safety-related inspection reports contair.cd similar changes.

This is a Severity Level V Violation. (SupplementII.E)

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby

required to submit to this office, within 30 days of the date of this Notice, a

written statement or explanation in reply, including: (1) the reason for the

violation if admitted; (2) the corrective steps which have been taken and the

resultsachieved;(3)correctivestepswhichwillbetakentoavoidfurther

violations; and (4) the date when full compliance will be achieved. Consid-

eration may be given to extending your response time for good cause shown.
