l NOTICE OF VIOLATION Gulf States Utilities Docket No. 50 458 River Bend Station Operating License: NPF-47 EA 89-122 l

During an NRC inspection conducted during May 1-5 and 15-19, 1989, a violation ,

of HRC requirements was identified. The violation involves the failure to establish a test program to assure the operability of the fuel building ventilation charcoal filtration subsystems and the main control room air conditioning system under all design basis conditions. In accordance with the

" General Stateuent of Policy and Procedure for NRC Enforcement Actions," 10 CFR ,

Part 2, Appendix C, 53 Federal Register 40019 (October 13,1988), the violation  !

Contrary to the above, in November 1988 it was identified that Gulf States Utilities (GSU) had failed to establish a test program at River Bend Station to assure that all testing required to demonstrate that structures, systems, and components will perform satisfactorily in service is identi- l fied and performed. Specifically, GSU failed to establish a test program ]

to demonstrate that two independent fuel building ventilation charcoal filtration subsystems and two independent main control room air handling unit / filter train subsystems would perform satisfactorily in service. As a result, GSU failed to discover design and installation flaws that would have prevented these subsystems from operating as intended under certain design conditions.

This is a Severity Level III violation. (Supplements)(458/8918-01)

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTH: Document Control Desk, Washington, D.C. 20555 with copies to the Regional Administrator, Region IV ano the NRC Resident Inspector at River Bend Station, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken f (3) the corrective steps that will be taken to avoid ,
