L APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458 River Bend Station License: NPF-47 During an NRC inspection conducted June 1-30, 1989, a vinlation of HRC l' requirements was identified. The violation involved inadequate system I

configuration control. In accordance with the " General Statement of Policy and

' Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed.below:

, Inadequate System Configuration Control River Bend Station Technical Specifications, paragraph 6.8.1.a requires that procedures for a'ctivities identified in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978, be established, implemented, and maintained. Paragraph 9 of Regulatory Guide 1.33 requires that maf ntenance l activities that can affect the performance of safety-related equipment be properly preplanned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the circumstances.

l Contrary to the above, Administrative Procedure ADM-0028, " Maintenance l Work Order," Revision 10, was not appropriate in that it did not require the maintenance work order (MWO) job plan to provide specific restoration steps if valve manipulations are performed or that the system be restored in accordance with the applicable ,'rocedure. As a result, 18 control rod drive hydraulic control units were not verified as being properly aligned following flush and vent activities authorized by MWO R126315. This condition existed from June 2-17, 1989.

This is a Severity Level IV violation. (Supplements)(50-458/8928-01)

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office, within 30 days of the letter transmitting this Notice, a written statenent or explanation in reply, including for each violation: -(1) the reason for the violation if-admitted, (2) the corrective steps that have been taken and the results achieved, (3 the corrective steps that will be taken to avoid further violations, and (4))the date when full compliance will be achieved. Where good cause is shown, consideration will be f given to extending the response tine.
