Dear Sir:

The NRC was notifi e d of a reportable deficiency in Reference A, an Interim Report was transmitted by Reference B, a Time Extension was requested by Reference (C), and a Final Report was transmitted by Reference (D).

The NRC was notified in Reference (E), that the corrective action specified in Reference (D) failed to correct the deficiency and the subject DER had been re-opened.

Due to the extensive investigation and evaluation required, a revised Interim Report is attached. It is now expected that this information will be finalized by July 20, 1984, at which time a complete report will be submitted.

INTERIM REPORT - REVISION 1 - DER 83-63 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC. SERVICE COMPANY (APS)

PVNGS UNITS 1, 2 Due to the additional scope of this condition, this interim report is being issued to supersede the final report, dated March 22, 1984.

I Potential Problem While attempting to open valves SIA-UV-617, SIA-UV-627, SIA-UV-637, SIA-UV-647, SIB-UV-616, SIB-UV-626, SIB-UV-636, and SIB-UV-646 against HPSI "A" & "B" pump flows the valves would not open. The valsa motor operators torqued out immediately after the bypass switch opened. The corrective action outlined in the final report has'been determired to be insuf ficient. Additionallys two other valves have been identified as having similar problems with the motor cperators. They are valves SIA-UV-666 and SIB-UV-667, HPSI pump mini-flow isolation valves.

The eight (8) High Pressure Safety Injection Isolation valves are Borg Warner Model No. 77620-2. The two High Pressure Safety Injection Pump 2ecirculation Isolation valves are Borg Warner Model No. 77620-4 (Unit 1) and 77620-3 (Units 2 and 3)'. All of the valves have the same model motor cperators (Limitorque SMC-04-7.5).

II. Approach To and Status of Proposed Resolution Bechtel Engineering is corresponding with C-E to obtain a complete disposition.

J III. Projected Completion of Corrective Action and Submittal of the Final Report
