ATTACHMENT 1 SURRY POWER STATION UNIT NO.1 DOCKET NO: 50-280 REPORT NO: 82-058/03L-0 EVENT DATE: 05/16/82 TITLE OF THE EVDIT: Radiation Monitor (RM-CC-105) Failure to Alarm

1. DESCRIPTION OF EVENT:

With Unit 1 at 100% power and Unit 2 shutdown for maintenance, performance of PT 26.1 (Radiation Monitoring Equipment Check) revealed that Component Cooling System Radiation Monitor, RM-CC-105, was reading slightly above the alarm setpoint but was not alarming. This is contrary to Technical Specification 3.13 and 2s reportable per Technical Specification 6.6.2.b.(4).

2. PROBABLE CONSEQUENCES OF OCCURRENCE:

The Component Cooling Radiation Monitor provides for the automatic closure of Component C 311ng Surge Tank Vent Valve (HCV-100) at alarm conditions.

The vent valve discharges to atmosphere through the Process Vent Radiation Monitors, R!!-GW-101 and 102. Since the process vent system was indicated, public health and saf ety were not aff ected.

3. CAUS E:

The failure of the radiation monitor to alarm at setpoint was the result of the alarm and oscillator cards drif ting out of calibration.

4. I}DfEDIATE CORRECTIVE ACTION:

Operations personnel performed the immediate actions of, AP-5.5, which requires closure of the C.C. Surge . Tank Vent Valve.

5. SUBSEQUENT CORRECTIVE ACTION:

The radiation monitor was inspected by the instrument technicians, who found the alarm and oscillator cards to be out of calibration. These cards were calibrated and the instrument was verified operable then returned to service.

6. ACTION TAKEN TO PREVENT RECURRENCE:

No further actions are required.

None.
