ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 2 i-I. Description of Deficiency

, During pipe spool alignment and installation 'to design location,_ Welds WOO 1 and WOO 2 of the Main Steau System Spools ISG033-S01 and S02 were temporarily supported at an incorrect location. To obtain alignment at Weld-WOO 4, the' temporary cribbing for' Weld WOO 3 was lowered

, to its design elevation in a cold condition. The re-sultant movement of the pipe could possibly cause ex-cessive stress between steam generator nozzle and Weld 003. The cause is attributed to'an accumulation of fit-up tolerances during installation of the various spools.

II. Analysis of Safety Implications The Engineering analysis of this condition has determined

, that this condition did not induce excessive stress in the piping system or steam generator-nozzle. Should this condition remain undetected, it would not have represented a safety significant condition.

t III. Corrective Action I

The NCR will be dispositioned as "Use-As-Is". The

responsible individuals have been consulted to perform complete fit-up inspections of piping assemblies prior to making the final welds.
