Groove Pin Dedication Equipment Affected by* this Information Notice:

Items subject to this Fisher Information Notice (FIN) were provided to Tennessee Valley Authority (TVA) Watts Bar Nuclear Plant per TvA Order 702507 (Fisher Order Number 025 -

N230331922). Specifically, this FIN applies to a plug/stem assembly that Fisher Controls International LLC (Fisher) supplied to TVA, item number 15A2560X072 (Equipment). In this Equipment, the essential-to-function groove pin was erroneously *provided as commercial-grade.

Purpose:

The purpose of this FIN is to alert the customer that, as of 22 January 2016, Fisher became aware of a situation which may affect the p~rformance of the aforementioned Equipment, including its safety-related function.

Fisher is informing the customer of this circumstance in accordance with Section 21.21 (b) of 10 CFR21.

Applicability:

This FIN applies only to the Equipment supplied by Fisher to TVA Watts Bar via the aforementioned order.

Discussion:

Thi? supplied Equipment in question features a plug tip which is connected to the plug by a simple threaded joint. A groove pin is used to prevent the plug tip from unscrewing from the plug block (please see attached Item 1for pa~ identification). Because the threaded joint is not self-locking, the groove pin serves as a primary locking device. It is therefore considered essential-to-function and requires dedication per FMP2K27 (Control of Commercial Grade Items to be Dedicated for Use in Nuclear Safety-Related Applications). However, review of the documentation package reveals that the groove pin was not subjected to the requirements of FMP2K27; thus, the part was supplied as a commercial'-grade item.

Th~ part in question does not pose an inherent safety risk and there are no known field issues with the affected Equipment.

Action Required:

Fisher will provide TVA with a properly dedicated groove pin. In addition, a Corrective Action Request (CAR 1796) has been initiated by Fisher to prevent reoccurrence of this issue.

10 CFR 21 Implications:
