The trip was due to a combination of events including a site modification which lowered the minimum speed of the main feedwater pump, operator actions to adjust the main feedwater pump speed bias and a procedure which was not specific enough. The procedure has been revised, the event will be included in the operations requalification training program, and the site modification program is bein evaluated.
