At 0600 hours0.00694 days <br />0.167 hours <br />9.920635e-4 weeks <br />2.283e-4 months <br /> on November 17, 1980, the 2P15A safety injection pump was taken out-of-service to repair a minor oil leak. The unit was operating at 100% power and

the redundant safety injection pump, 2P15B, was tested operable prior to removing 2BlSA from ' service. While working on the pump, an inspection by the mechanic found the coupling-to-shaft key on both the motor end and pump end were sheared.

Problems were encountered while trying to repair the pump

because the coupling mechanically bound to the sha5t by the i

sheared keys and could not be hydraulically jacked off the shaft. Both the motor and pump shaf ts had to be cut and bored out of the coupling. These problems extended the repair time beyond the 24-hour time limit allowed by Technical Specification 15.3. 3.2.a for operation with one safety injection pump out-of-service, and an orderly shutdown was initiated at 0600 hours0.00694 days <br />0.167 hours <br />9.920635e-4 weeks <br />2.283e-4 months <br /> on November 18, 1980. Repair of the safety injection pump was completed and the pump satisfactorily tested at 1801 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.852805e-4 months <br /> on November 18, 1980. The unit returned to line at 1905 hours0.022 days <br />0.529 hours <br />0.00315 weeks <br />7.248525e-4 months <br /> on November 18, 1980, but did not achieve full power until 0735 hours0.00851 days <br />0.204 hours <br />0.00122 weeks <br />2.796675e-4 months <br /> on November 20, 1980,

because of delta flux limitations.

The exact time the keys were sheared is not known; however, the pump was evaluated to have been operable prior to being taken out-of-service on November 17, 1980. This is

'

substantiated by the fact that the pump performed satis-l factorily during all monthly and annual inservice and system

[ tests. Further the coupling was so securely bound to the shaf ts that it could not be hydraulically removed. Either l

i nechanical or friction fusion which prevented removal 'of the I coupling would have continued to directly tie the motor. to the pump for'any eventual use.

Discovery of the sheared keys. and the required plant shutdown are reportable in accordance with Technical Specification 15. 6. 9. 2.B. 2.

The resident inspector was notified orally on November 17, 1980.
