Radiation Safety Baseline Inspection Initial Information Request Inspection Report: 2019001 During the weeks of February 4- 8 and February 25 - March 1, 2019, the NRC will perform a baseline Radiation Safety Inspection at Hatch Nuclear Plant (NRC Inspection Procedures 71124.01, 71124.02, 71124.03, 71124.04, 71124.05 and the radiation safety sections of 71152).

Experience has shown that this inspection is resource-intensive for both the NRC inspectors and your staff. In order to minimize the impact to your onsite resources and to ensure a productive inspection, we are requesting in advance documents needed for this activity. It is important that all of these documents are up-to-date, and complete, thereby minimizing the number of additional documents requested during the preparation, and/or the onsite portions of the inspection. The NRC requests that these documents be provided to the inspectors no later than January, 2019.

Please organize the information as it is arranged below to the extent possible. Experience has shown that a poorly organized CD leads to a less efficient inspection and places additional burden on licensee staff. Pay particular attention to the date ranges for the items requested as they may change from item to item. If there are questions regarding the documents requested, or if the documents cannot be provided by the due date, please do not hesitate to contact the lead inspector.

Documentation for the inspection procedures from March 1, 2017 to the present is requested for all procedures, except 71124.01 and 71151, which should be from March 1, 2018 to present. This reflects the last time these areas were inspected. We would prefer as much of the information as possible in electronic form. An index to the CD contents is also helpful. For those items requesting a list of documents/areas, the inspector will select documents/areas from the list for on-site review.

71124.01 - Radiological Hazard Assessment and Exposure Controls (Last Inspected February 2018)

1. List of active routine and 1R21 outage Radiation Work Permits (RWPs).
2. Copies of RP Procedures for access and key controls, posting, labeling, surveys, RWPs and ED setpoints, contamination control, release of material and personnel from the RCA, diving, high risk activities and any RP procedures for specific plant evolutions (eg.

Crudburst, Resin Transfer, Rx Head Movement, etc.).

3. Copies of the last two routine surveys from the ISFSI facility, U1 & U2 RHR rooms and Containment Penetration Rooms.
4. Copy of radioactive source inventory, most recent leak test survey and any National Source Tracking System (NSTS) transactions since March 1 2018.
5. List of all non-fuel items stored in spent fuel pool.
6. List of CAP report numbers and brief descriptions for issues related to RP controls, radworker or RP tech human performance, posting and surveys generated since March 1, 2018.

71124.02 - ALARA Planning and Controls (Last Inspected February of 2017)

1. Site and/or corporate ALARA Program and all associated procedures for maintaining dose ALARA including permanent and temporary shielding processes.
2. List of top five dose jobs for the upcoming U2 outage and associated ALARA planning packages.
3. ALARA planning and reviews conducted for the last ISFSI campaign
4. 2018 annual ALARA report and most recent refueling outage report.
5. Completed ALARA packages for the last outage for five work activities that had the greatest collective dose, and/or presented significant radiological risk.
6. ALARA goals for 2019.
7. ALARA Committee meeting minutes associated with HNP2 preps.
8. Plant source term reduction strategy and results achieved since March 1, 2017.
9. List of CAP report numbers and brief descriptions generated since March 1, 2017 related to the ALARA program.

71124.03 - In-Plant Airborne Radioactivity Control and Mitigation (Last Inspected February of 2017)

1. Site and corporate procedures/manuals associated with airborne radiation monitoring instrumentation use, maintenance and setpoint determination.
2. Site and corporate procedures/manuals for all plant respiratory protection equipment in use.
3. Copies of vendor certifications for plant personnel qualified to service SCBA equipment if applicable.
4. Records of certification of air quality for equipment used to provide breathing air for air-supplied respirators and/or filling SCBA bottles since March 1, 2017.

Two most recent surveillances on the installed ventilation systems for the Refueling Zone HVAC System and the Main Control Room Environmental Control System (MCRECS) including flow verifications and filter efficiencies as well as the basis for setpoints for the radiation monitors in these systems/facilities:

71124.04: Occupational Dose Assessment (Last Inspected February of 2017)

2. The plants prospective evaluation, if applicable, determining if monitoring is required by 10CFR 20.1502.
3. The last two DLR/ED correlations.
4. On site passive area radiation monitoring data/evaluations demonstrating compliance with 10 CFR 20.1502 for unmonitored personnel.
5. Most recent evaluations of neutron spectra for the ISFSI and containment at power.
6. List of individuals and exposures for individuals receiving >500 mrem TEDE, >100 mrem neutron, >10 mrem CEDE and >500 mrem SDE in 2017 and 2018.
7. National Voluntary Laboratory Accreditation Program (NVLAP) accreditation documentation for current dosimetry used by the site.
8. List of all personnel contamination events, dispersed contamination/discrete particles, identified since March 1, 2017. [Note: only a listing should be provided for use by the inspectors to select a sample of issues for in-depth review during the onsite inspection].

71124.05 - Radiation Monitoring Instrumentation (Last Inspected February 2017)

1. Provide a list of current portable instruments including electronic dosimetry on site and the procedures/guidance documents for their use, issue and calibration.
2. Procedures/Guidance documents for use and calibration of countroom and fixed contamination monitoring equipment and the sites evaluation of the internal dose sensitivity of the exit portal monitors.
3. Procedures/Guidance and the most recent calibration of the portable instrument calibrator.
4. The last two records of WBC calibration.
5. Interlaboratory crosscheck results for countroom equipment
6. The last two calibration records for the Unit 2 Drywell High-range Area Monitor and calibration source documentation including paperwork showing traceability to a National Institute of Standards & Technology (NIST) standard and/or traceability to the primary calibration, as applicable.
7. List of the area radiation monitors and setpoints currently in service. Several will be selected for on-site review of the calibration records.

71151 - Performance Indicator (PI) Verification (Last Inspected February 2018)

1. Site procedures/manuals for gathering and reporting PI data.
2. Monthly/Quarterly PI reports since March 1, 2018, and copies of associated corrective action reports for any RETS/ODCM Radiological Effluent occurrences.
3. List of all corrective action documents since March 1, 2018, using keywords such as: HRA, LHRA, VHRA, unintended dose, unlocked door, etc.
4. List of all electronic dosimeter (ED) dose rate alarms and ED dose alarms since November 1, 2017 which includes dose or dose rate alarm received, and the alarm setpoint(s).
5. List of condition reports (e.g. NCRs, CRs, ARs etc.) generated since March 1, 2018, using keywords abnormal/ unmonitored effluent release, etc.
