SUMMARY

REP(In accordance with the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code, Section XI, Article IWA-6230, TVA is providing the SQN ISI Summary Report within 90 days from completion of the inspections performed during the UlCI4 refueling outage. The summary report contains an overview of the in-service examinations and augmented non-destructive examination results that were performed on ASME Class 1 and 2 components from November 20, 2004, to May 15, 2006. This report also contains a summary of steam generator tube examinations (Appendix A), a report of the repair and replacement activities (Appendix B), a pressure test report (Appendix C), and the IWE metal containment evaluations (Appendix D). This report completes the ASME Section XI examinations for ASME Code Class 1 and 2 components for SQN's second 10-year interval.This report does not contain TVA commitments. This report also includes repairs and replacements and pressure tests performed during this period. The Unit 1 Cycle 14 Refueling Outage began when the generator was taken off line on April 10, 2006. The outage was completed on May 15, 2006, when the generator was tied to the power grid. The inservice examinations, which include risk-informed inservice inspection examinations were, performed to the implementing plant Surveillance Instruction, 0-SI-DXI-000-1 14.2, "ASME Section Xl ISI/NDE Program Unit 1 and Unit 2" revisions 21 thru 28. The steam generator tubing eddy current examinations are discussed in Appendix A. Repairs and replacements are discussed in Appendix B. Pressure test examinations are discussed in Appendix C. The IWE metal containment evaluations are discussed in Appendix D. Use of code paragraph IWC-1 221 (e) reduced the total number required for examination in examination category C-F-1 from 146 to 142 in the U1C9 report.6. Removed from credit 2 supports (1-SIH-031 and 1-SIH-160) reported in examination category F-A in UlC8 in the U1C9 report.7. Due to the support modifications in the second period, the total number of required examinations in examination category F-A for the interval decreased from 204 to 202 during U1C10 in the U1C10 report.8. The RI-ISI examinations are performed under examination category R-A item numbers R1.11, R1.12, R1.16 and R1.18. The nozzle-to vessel weld examination, Examination Category C-B, Item Number C2.20, .for the residual heat removal (RHR) heat exchanger will be used to satisfy the Item Number C2.21 RHR heat exchanger nozzle inside radius section examination. See request for relief 1-1S1-15. Two RHR heat exchanger nozzle inside radius sections were added to the total number of examinations credited for the interval for tracking. These debonding areas were determined to be minor due to the depth of the moisture barrier being approximately 6 inches. The SCV at these areas were VT-3 examined and there were no visible signs of active corrosion, any significant wall loss or gross degradation. The exposed SCV areas requiring recoating were recoated and VT-3 examined. The removed moisture barrier material was filled in with RTV and VT-3 examined.This minor debonding has not affected the integrity of the moisture barrier. These areas are normally inaccessible due to the stainless steel flashing and insulation (refer to drawing CISI-1000-C-59). Based on the information above, the moisture barrier areas are not considered suspect and do not impact the structural integrity or leak tightness of the SCV. No detrimental flaws were observed. The moisture barrier areas are acceptable for continued service, and no further corrective action is required. An additional section MB-1 of the moisture barrier was examined and debonding of the moisture barrier was identified. The moisture barrier was replaced in U1C10 and moisture barrier section MB-2 was examined in the last outage U1 C1 3 and no debonding of the moisture barrier was identified. These areas examined were identified for coating repairs during cleaning for the ultrasonic examination, and this visual examination (VT-3) was performed prior to surface preparation to satisfy the requirements of IWE-2500(b). Flaking paint and light corrosion were removed to allow for the ultrasonic examination. These areas are not considered suspect and do not impact the structural integrity or leak tightness of the SCV. 3. 8 at elevation 730 each had a complete weld on the other side and are acceptable by providing significant load path through the connection.

The metal damage on the outside plate to the horizontal stiffener B and between vertical stiffeners
