SECOND INTERIM REPORT Descriution of Deficiency During a TVA audit, a number of deficiencies were identified against the Penn Ventilator Co. QA program. The deficiencies indicated that Penn was not implementing the QA manual that had been submitted to TVA and approved for use.
Corrective Action TVA will review available documentation demonstrating the adequacy of the equipment. Interface submittal drawings were not adequate for verification purposes, and therefore, detailed shop assembly and design drawings of the ventilators have been requested from Penn Ventilator Co.
TVA has received approved c:ocumentation for the seismic and environmental qualification of the ventilator componet.ts and a certificate of conformance from a seismic consultant.
Upon receipt of the detailed drawings from Penn Ventilator, TVA will review the drawings and the seismic and environmental qualification documentation for compliance with the specifications. TVA will further inspect the actual equipment for compliance with the documentation.
