Contrary to the above, the tinit was heated up to Mode 3 on July 9, 1980 with the Containment Spray System and the steam driven Auxiliary Feedwater Pump inoperable. On July 14, 1980, valves 21CS6 and 22CS6 were 'found, tagged closed., renderi~g the Containment Spray System inoperable *c.Technical Specification 3.6.2.1). On July 13, 1980 valves 21MS45 and 23MS45 were *found, tagged closed, rendering the steam driven Auxiliary Feedwater Pump inoperable . {Technical Specification 3. 7 .1. 21. In each case I the tagging predated the entry into Mode 3.

The discovery was made prior to required operability of the respective instruments in accordance with Technical Specification 3.3.2.1. Due to the potential loss of required instrumentation, licensee corrective action to prevent recurrence is being requested.
