We have reviewed the report of your in~pection conducted *by Mr. L. Norrholm during the *period July 1-31, 1980. Our response*

to the item of noncompliance identified in Append-ix A a,nd the item of concern addressed in your report transmittal letter is as follows:

Item: 1, r:nfraction Technical Specification 3. O. 4 states, "Entry into. a,n Ol?EMTIONAL MODE or other specified applicability condition sha;ll not be* made unless the *conditions of the Limiting Condition for Operation are met without reliance on provisions contained in the ACTION state-ments unless otherwise excepted."

Contrary to the above, the tinit was heated up to Mode 3 on July 9, 1980 with the Containment Spray System and the steam driven Auxiliary Feedwater Pump inoperable. On July 14, 1980, valves 21CS6 and 22CS6 were 'found, tagged closed., renderi~g the Containment Spray System inoperable *c.Technical Specification 3.6.2.1). On July 13, 1980 valves 21MS45 and 23MS45 were *found, tagged closed, rendering the steam driven Auxiliary Feedwater Pump inoperable . {Technical Specification 3. 7 .1. 21. In each case I the tagging predated the entry into Mode 3.

1. The cause was an improper use of a temporary on-the-

. spot change, as applied to a .surveillance procedure.

2. To correct the situation, the tags were released and

~he valves were locked open. *

3. To prevent recurrence, the Operating Department pro-cedure *concerning the use of on-the-spot changes will be 'revised. This revision will prohibit the use of a temporary on-the-spot change to modify any surveillance restoration step. A Night Order Book entry was made to emphasize the proper use of the on-the-spot change.

4. Date for full-compliance is November 1, 1980.

B. 21/23MS45

1. The cause was a personnel error. The identified tags

.were released prior to entering Mode 3, however, the tags were not immediately removed nor the valves repositioned.

2. To correct the situation, the tags were removed and the valves were locked open. *

3. To prevent recurrence, the administrative procedure for the Ta9ging System (AP-15} is being revised to require a log entry by the Senior Shift Supervisor when a tagging request is not released upon sign-off by the supervisor completing the work. *

4. Date for procedure revision is November 1, 1980.

Ttem 2,* unresolved *Item No~* 31"1/8*0*-*12:-:02 Heatup of the unit began on July 7, 1980 and Mode 4 established.

The unit entered Mode 3 *(Hot Standby) on July 9. Cooldown to Mode 4 was initiated when it was discovered that several instru-ment channels were not operable and tracking plant conditions l3 steam flow channels,* one steam pressure *channel, and two pres-surizer pressure *channels)

• These channels were later found to be isolated. due to shut isolation valves. The discovery was made prior to required operability of the respective instruments in accordance with Technical Specification 3.3.2.1. Due to the potential loss of required instrumentation, licensee corrective action to prevent recurrence is being requested.

• Reply to Item 2 These identified items were addressed in the follow:i.ng LERs:

80-10/03L (Pressurizer Pressure} and 80-09/03L (Steam Flow and Pressure).

The LER investigations revealed that these were isolated inci-dents. We feel the corrective action conunitted to in the associ-ated LERs is sufficient to prevent recurrence and no further action is anticipated.
