The information is as initially received without verification or evaluation, and is basically all that is known by the Region IV staff on this date.

PLANT SHUTDOWN GREATER THAN 48 HOURS The SRI and licensee reported that questionable lug crimping in "Limitorque" valve operators was identified on June 3, 1986, during continuing evaluations being performed by the licensee due to previously identified problems with the "Limitorque" valve operators (wiring type, size, and splices). The licensee commenced plant shutdown, attaining the Hot Standby condition on June 4,1986, at about 2 a.m. CDT. The licensee evaluations and corrective actions associated with about 156 safety-related valve operators is continuing and the licensee plans about a 5-day outage at this time to complete the activities.
The licensee plans to issue a press release.
The state of Kansas will be informed.
Region IV received notification of this matter by telephone from the SRI and licensee on June 3, 1986, at approximately 4 p.m. CDT. This information is accurate as of about 11:30 a.m. CDT, June 4, 1986. Region IV has informed IE/NRR/SP/PA.
