The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

SUBJECT:
V0ID IN REACTOR CONTAINMENT WALL UNDER EQUIPMENT HATCH The Kansas Gas & Electric Company notified Region IV of a void in a containment wall concrete placement. The forms for the sixth lift wall placement (10 ft. per lift) were moved up in preparation for the next placement on December 13 uncovering the void area. The void under the equipment hatch was reported to reach thru the wall and extended several i
feet on either side of the hatch.

The void is considered repairable by the licensee and has been ' identified as a nonconforming item.
The repairs will commence after review by the

r The licensee is evaluating the situation for possible reporting as a construction deficiency.
A Region IV investigation team will be on site Monday, December 18, to investigate this and other concrete problems.
TV coverage on site has been made by WIBW (Topeka).
High interest is expected due to base mat problems covered the previous day by the media.
The Region IV (Arlington, Texas) inspector received notification of this matter at approximately 2:30 p.m. on December 14,1978.
