Description:

MOV-'SW-103B Valve Non-function With the unit at refueling shutdown-, MOV-SW-103B did not operate electrically during a flush of the Recirc Spray heat exchangers. This is contrary to T.S.

3.14.A.6 and is reportable per T.S.-6.6. 2. b(2).

2. Probable Consequences:

MOV-SW-103B admits service water to the header which supplies cooling~ water to the Recirc Spray heat exchangers. This valve is one of four redundant valves that will supply

the Recirc Spray heat exchangers with service water.

The redundant MOV-:'s were shown to be functional, therefore the health and safety of the general public were not affected.

3. Cause:

There were no electrical or mechanical deficiencies found with MOV-SW-103B when it was inspec-ted subsequent to the aforementioned event. This valve is only cycled during refueling shutdown due to operational restrictions associated with Recirc Spray heat exchangers a?d* therefore could have been

  • Immediate Actions:

The immediate actions were to cycle the valve manually (following this cycling the valve operated electrically) and submitt an MR to have the valve inspected.

5. Subsequent Corrective Action:

The subsequent corrective action was to.dissemble and clean the valve.

6. Action to Prevent Recurrence:

According to past records, there have not been a similar occurance involving MOV-SW-103A,B,C, or D, this appears to be an isolated case, therefore no future action is required .
