IWith the Unit operating steady state at 100% power, it was found that TV-SS-201B would

DOCKET NO: 50-281 REPORT NO: 80-016/031-0 EVENT DATE: 09-16-80 TITLE OF REPORT: TRIP VALVE TV-SS-201B MALFUNCTION

1. Description of Event:

With the unit operating steady state at 100% power, .it was found that TV-SS-201B would not remain closed when the control switch was released. This is contrary to T.S.-3.8:A.l and is reportable per T.S.-6.6.2.B(2).

2. Probable Consequences:

If isolation of this line had been required, the inside trip valve (TV-SS-201A) would have closed. Containment int~grit'y would therefore have been maintained and the health and safety of the general public were not affected.

3. Cause:

The inspection of the Trip Valves limit switches and associated components did not reveal any problems. The valve was successfully cycled and the malfunction could not be repeated. Therefore the exact cause cannot be determined.

4. Immediate Corrective Action:
  • 5.

The immediate corrective action was to close the inside trip valve and secure air to TV-SS-201B.

Subsequent Corrective Action:

  • The elcetricians inspected TB-SS-201B, cycled it satisfactory, and returned the valve to service.
6. Action Taken to Prevent Recurrence:

None
