Designation of Apparent Cause of Occurrence: Component failur; is the cause of this occurrence. The grab sampler is permanently mounted only cn stack radiation monitor RE 2024 which was inoperable for the week. The redundant stack monitor RE 2025 does not have a grab sampler installed.

Description of Occurrence: At 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> on August 30, 1977, while performing a routine surveillance test, ST 5099.06, " Effluent Radiological Monitoring", the Chemistry and Health Physics Tester discovered the net flow through the grab sample 3

Corrective Action: Stack radiation monitor RE 2024 was returned to service on September 3, 1977, returning the capability of obtaining weekly grab samples. A mcdification (T-2266) has been written to allow use of installed equipment on RE 2025 for an estimate of activity released if the incident re-occurs. Also, a Facility Change Request (FCR 77-314) has been written to request a grab sample unit be installed on RE 2025.

Failure Data: No previous similar events have occurred.
