On July 16,1980, at 1750 an alarm came up in the Control Room indicating vacuum breaker A0-1-1601-20B was open when it should be closed. The control switch open/ closed indicator lights were out. Check valve 1-1601-31B was locked closed ensuring primary containment integrity in accord-ance with Technical Specification 3.7.A.3.b.

VI. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The vacuum breaker is an air operated butterfly valve allowing air to be vented f rom the reactor building to the torus, preventing external pressure exceeding primary containment pressure by more than two psi.

Reverse flow is prevented by a check valve. The 100 percent redundant A0-1-1601-20A vacuum breaker remained in operation while the 20B vacuum breaker was repaired.

Vll. CAUSE:

The cause of this occurrence is equipment failure. The coil in air actuator solenoid 50-1-1601-20B manufactured by ASCO burned out leaving vacuum breaker A0-1-1601-20B in the open position.

Vill. CORRECTIVE ACTION:

Check valve 1-1601-31B was locked closed maintaining primary containment until maintenance replaced the defective coil. The vacuum breaker is operating correctly giving the proper indication on the control panel.
