In accordance with Technical Specification 6.9.1.8.f, the following occurrence is reported. The incident was reported to Mr. L. H. Thonus of Region I at approximately 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br /> and Mr. D. A. Beckman, resident inspector, at approximately 1545 hours0.0179 days <br />0.429 hours <br />0.00255 weeks <br />5.878725e-4 months <br /> on November 27, 1979.

The flow path from the refueling wo.e. storage tank to the high head safety injection pumps is via a path controlled by parallel motor-operated isolation valves which are normally closed. At 0637 hours0.00737 days <br />0.177 hours <br />0.00105 weeks <br />2.423785e-4 months <br /> on November 27, the No. 1 Emergency Diesel Generator was removed from service for scheduled preventive maintenance. The generator provides emergency power to [CH-115B],

one of the two parallel motor-operated isolation valves on the RWST. At about 0900 hours0.0104 days <br />0.25 hours <br />0.00149 weeks <br />3.4245e-4 months <br />, the redundant motor-operated isolation valve [CH-ll5D} was placed on clearance, in the closed position, to correct a motor-operator malfunction.

The flow path to the charging pumps would have been available with offsite power available. In the event of a simultaneous requirement for high head safety injection and a loss of offsite power, high head injection would only have been available until the Volume Control Tank emptied. The incident was caused when clearance approval was granted in the midst oT '"

normal early morning demands on operator time.

The valve was opened at 1029 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.915345e-4 months <br /> providing the required flow path to the safety injection pumps.

Any additional information and corrective action will be discussed in the followup report.
