the following Special Report is submitted.

Site Administrative Procedure 9D requires the repair of an identified discrepancy within fourteen days and a Special Report within the following thirty days if the discrepancy is not resolved within the fourteen day period.

This report is being submitted because the corrective actions were not completed within the initial fourteen day time period after discovery.

On March 5,

1990, during the performance of trip' testing of fire dampers under air flow conditions, East Cable Vault Exhaust Damper VS-D-180 failed to fully close.

VS-D-180 is a Category F damper located in the ceiling (elevation 756') between the East Cable Vault and the Motor Control Center Room.

Air is ' withdrawn from the East Cable Vault using the Supplementary Leak Collection and Release System.

The East Cable Vault utilizes a Carbon Dioxide System to provide fire suppression.

The failure of this damper to close ander air flow conditions, degrades the carbon dioxide fire suppression:for the East Cable Vault.

The cause for the event was an inadequate spring closure mechanism.

The spring closure assembly has been upgraded with helical coil springs and torsion springs to the blade ends to improve the damper closure.

These actions could not be completed during the fourteen day time period as the necessary parts were required to be ordered and were not received during that time period.

This is an interim corrective action until this damper is replaced during the Eighth Refueling Outage, scheduled for April 1991.

A fire watch had previously been established in the East cable Vault due to identified problems with the Underwriters Laboratory qualification for this and other dampers in that area (

Reference:

Special Report ND3MNO 1953, dated September 15, 1989 end LER 89-010).

These previous, non-similar problems involved improperly field-fabricated dampers that did not meet the required fire rating for the East cable Vault Area.
